【摘要】：目的:探討雙波長(zhǎng)激光對(duì)比鹽酸米諾環(huán)素治療牙周牙髓聯(lián)合病變的差別及牙周袋菌群分析。方法:收集牙周牙髓聯(lián)合病變患者60顆患牙,隨機(jī)分為2組:實(shí)驗(yàn)組Nd:YAG消毒根管一次性充填根管+Er:YAG處理牙周袋;對(duì)照組碘伏消毒根管一次性根管充填+鹽酸米諾環(huán)素牙周袋上藥。按視覺(jué)模擬評(píng)分法(visual analogue scale,VAS)觀察兩組治療后1d疼痛差異,對(duì)比兩組治療1周、2周、3周、3月、6月、1年的牙周袋深度(the depth of periodontal pocket,PD)、牙周附著水平(clinical periodontal attachment level,CAL),探診出血指數(shù)(bleeding on probing,BOP),評(píng)價(jià)療效。抽取兩組病例各4例,并定期采集患牙牙周袋菌斑行高通量測(cè)序。結(jié)果:實(shí)驗(yàn)組與對(duì)照組治療1d疼痛有差異;不同時(shí)間點(diǎn)間PD、CAL靜息狀態(tài)下VAS評(píng)分有差別;BOP陽(yáng)性位點(diǎn)發(fā)生率有差別。2組牙周袋菌斑多樣性差異不大,以韋榮氏菌、變性鏈球菌、卟啉單胞菌、消化鏈球菌為主。結(jié)論:雙波長(zhǎng)激光聯(lián)合輔助治療牙周牙髓聯(lián)合病變的效果優(yōu)于鹽酸米諾環(huán)素,具有較好的臨床應(yīng)用前景。
[Abstract]:Objective: to investigate the difference of the treatment of periodontal pulp syndesmosis with minocycline hydrochloride by dual-wavelength laser and to analyze the bacterial flora of periodontal pouch. Methods: 60 teeth with periodontal pulp syndesmosis were collected. The experimental group was randomly divided into two groups: the experimental group was treated with Nd:YAG by filling the root canal with Er:YAG at one time. In the control group, minocycline hydrochloride was filled with minocycline hydrochloride periodontal bag at one time. The difference of pain between the two groups was observed 1 day after treatment by visual analogue score (VAS). The results were compared between the two groups in 1 week, 2 weeks and 3 weeks, March, June, 1 year after treatment with the depth of periodontal pocketn, the periodontal attachment level and the clinical periodontal attachment level, and the bleeding index, bleeding-on probing, BOPN, respectively. 4 cases in each group were selected, and 4 cases in each group were selected, and 4 cases in each group were selected, and 4 cases in the two groups were selected, and 4 cases in each group were selected, and 4 cases in each group were selected. High throughput sequencing of periodontal pouch plaque was performed. Results: there was a difference between the experimental group and the control group in 1 day pain. There was no significant difference in the incidence of VAS positive loci between different time points. 2 groups had no significant difference in the diversity of periodontal pouch plaque, such as Veron's bacillus, Streptococcus mutans, porphyromonas porphyromonas. Conclusion: Dual-wavelength laser assisted treatment of periodontal pulp syndesmosis is superior to minocycline hydrochloride in clinical application.
