【摘要】：目的:探討伴抑郁癥的功能性消化不良(FD)患者的人格特點及可能的相關因素。方法:本研究為橫斷面研究。在北京協(xié)和醫(yī)院消化內科和心理醫(yī)學科門診經雙向轉診完成臨床診斷,共納入126人,分為三組:FD組為符合羅馬III FD標準,不符合美國精神障礙診斷與統(tǒng)計手冊第4版(DSM-IV)抑郁癥標準的患者(n=43);抑郁癥組為符合DSM-IV抑郁癥標準,不符合羅馬III FD標準的患者(n=35);FD伴抑郁癥組為同時符合以上FD和抑郁癥診斷標準的患者(n=48)。采用艾森克人格問卷(EPQ)、漢密頓抑郁量表-17項(HAMD-17)、漢密頓焦慮量表(HAMA)和蒙哥馬利抑郁量表(MADRS)分別評估患者的人格特征和抑郁焦慮水平。結果:FD伴抑郁癥組的EPQ精神質[(48.7±7.9)vs.(43.6±8.1)]和神經質[(64.3±9.6)vs.(56.9±13.4)]得分高于FD組(P0.01),外向性得分低于FD者[(47.1±11.3)vs.(54.0±14.2),P0.05]。FD患者的HAMD-17、HAMA和MADRS總分與EPQ精神質和神經質得分呈正相關(r=0.25~0.62,均P0.05),與外向性程度呈負相關(r=-0.407~-0.431,均P0.05)。結論:功能性消化不良共病抑郁癥的患者可能存在較明顯的精神質、內向性和神經質人格特點。
