血漿同型半胱氨酸水平與血管性認(rèn)知損害及海馬體積的相關(guān)性研究

【摘要】:目的探討高同型半胱氨酸(Hcy)血癥與海馬體積及血管性認(rèn)知功能損害的相關(guān)性。方法采用病例對(duì)照研究,納入腦梗死患者194例,所入選患者均測(cè)定血漿Hcy濃度,用腦磁共振三維測(cè)量方法測(cè)量海馬體積,采用蒙特利爾認(rèn)知評(píng)估量表(MoCA)進(jìn)行認(rèn)知測(cè)定,根據(jù)血漿Hcy水平分為正常Hcy組(血漿Hcy15μmol/L,49例);Hcy輕度升高組(血漿Hcy 16~30μmol/L,78例);Hcy中重度升高組(血漿Hcy 31~100μmol/L,52例);Hcy重度升高組(Hcy100μmol/L,15例)。結(jié)果血漿Hcy不同水平組間認(rèn)知功能MoCA總分差異有統(tǒng)計(jì)學(xué)意義(P0.05),血漿Hcy不同水平組間左、右側(cè)海馬體積差異有統(tǒng)計(jì)學(xué)意義(P0.05),血漿Hcy水平與認(rèn)知功能MoCA總分呈負(fù)相關(guān)(r=-0.504,P0.05),血漿Hcy水平與左、右側(cè)海馬體積均呈負(fù)相關(guān)(r=-0.472,P0.05;r=-0.647,P0.05)。左、右側(cè)海馬體積均與認(rèn)知功能MoCA總分呈正相關(guān)(r=0.569,P0.05;r=0.671,P0.05)。結(jié)論高Hcy血癥可能與腦梗死患者的海馬體積縮小及認(rèn)知損害相關(guān)。
[Abstract]:Objective to investigate the relationship between hyperhomocysteinemia and hippocampal volume and vascular cognitive impairment. Methods A case-control study was conducted in 194 patients with cerebral infarction. Plasma Hcy concentrations were measured, hippocampal volume was measured by three-dimensional cerebral magnetic resonance imaging (MRI), and cognitive measurement was carried out by Montreal Cognitive Assessment scale (MoCA). According to the level of plasma Hcy, they were divided into normal Hcy group (plasma Hcy15 渭 mol / L, 49 cases). Hcy mild elevation group (plasma Hcy 16 ~ 30 渭 mol / L, 78); Hcy moderate to severe elevation group (plasma Hcy 31 ~ 100 渭 mol / L, 52); Hcy severe elevation group (Hcy100 渭 mol / L, 15 cases). Results there was significant difference in the total score of cognitive function MoCA between different levels of plasma Hcy (P 0.05), and there was significant difference in left and right hippocampal volume between different levels of plasma Hcy (P 0.05).
