精神分裂癥患者血清同型半胱氨酸與注意及執(zhí)行功能的相關(guān)性

【摘要】:目的:探討血清同型半胱氨酸(Homocysteine,Hcy)與精神分裂癥注意及執(zhí)行功能的相關(guān)性。方法:選取門診及住院精神分裂癥患者118人,在該院康復(fù)科社會(huì)志愿者中選取健康者72例為正常對(duì)照組。測(cè)定空腹靜脈血清同型半胱氨酸(15μmol/L為高Hcy)、葉酸、維生素B12含量;采用連續(xù)操作測(cè)驗(yàn)(CPT)評(píng)定持續(xù)注意能力,威斯康星卡片分類測(cè)驗(yàn)(WCST)評(píng)定執(zhí)行功能。結(jié)果:患者組血清Hcy值高于正常對(duì)照組[(21.8±7.7)μmol/L vs.(15.4±5.2)μmol/L,P0.05],男性患者血清Hcy值高于女性患者[(27.9±4.4)μmol/L vs.(16.5±3.5)μmol/L,P0.05];颊呓M中高血清Hcy者共61人(51.69%),男性多于女性(61.6%vs.40.4%,P0.05);颊呓M的CPT和WCST測(cè)驗(yàn)得分均低于正常對(duì)照組,高Hcy患者的CPT和WCST測(cè)驗(yàn)得分均低于正常Hcy患者(均P0.05);颊呓M的血清Hcy值與CPT正確反應(yīng)數(shù)及WCST正確測(cè)驗(yàn)數(shù)、分類完成數(shù)得分均呈負(fù)相關(guān)(r=-0.83、-0.78、-0.74,均P0.01),而與CPT干擾項(xiàng)錯(cuò)誤數(shù)及WCST持續(xù)錯(cuò)誤數(shù)、隨機(jī)錯(cuò)誤數(shù)得分呈正相關(guān)(r=0.65、0.84、0.78,均P0.01)。結(jié)論:本研究提示,精神分裂癥患者的血清Hcy值異常升高與其注意及執(zhí)行功能密切相關(guān)。
[Abstract]:Objective: to investigate the relationship between serum homocysteine (Homocysteine,Hcy) and attention and executive function in schizophrenia. Methods: 118 out-patient and inpatients with schizophrenia were selected and 72 healthy volunteers were selected as normal control group. Fasting venous serum homocysteine (15 渭 mol / L, high homocysteine), folic acid and vitamin B12 were measured, continuous operation test (CPT) was used to evaluate persistent attention, Wisconsin card sorting test (WCST) was used to evaluate executive function. Results: the serum Hcy level in the patient group was higher than that in the normal control group [(21.8 鹵7. 7) 渭 mol/L vs. (15.4 鹵5. 2) 渭 mol/L,P0.05]. The serum Hcy level in male patients was higher than that in female patients [(27.9 鹵4.4) 渭 mol/L vs., (16.5 鹵3.5) 渭 mol/L,P0.05]. There were 61 patients (51.69%) with high serum Hcy in the patient group, and the male was more than the female (61.6% vs. 40.4%, P0.05). The scores of CPT and WCST in the patients group were lower than those in the normal control group, and the scores of CPT and WCST in the patients with high Hcy were lower than those in the normal Hcy patients (P0.05). In the patient group, the serum Hcy value was negatively correlated with the correct response to CPT, the correct test number of WCST, and the score of classification completion (r = 0.83, 0.78, 0.74, both P0.01), while it was negatively correlated with the number of CPT interference item errors and the number of WCST persistent errors. The scores of random errors were positively correlated (r = 0.65, 0.84, 0.78, P 0.01). Conclusion: this study suggests that the abnormal increase of serum Hcy level in schizophrenic patients is closely related to their attention and executive function.
