【摘要】：目的采用靜息態(tài)f MRI及行為學(xué)方法評價酒精依賴者靜息態(tài)腦功能改變及情緒記憶特點。方法共納入23例男性酒精依賴者,20例男性健康對照,采用E-prime軟件編寫程序,用聯(lián)想筆記本電腦對所有受試者采集行為學(xué)數(shù)據(jù),行為學(xué)結(jié)果采用單因素方差分析(one-way ANOVA)及多重比較分析。再通過3.0T MR儀器采集兩組受試者的靜息態(tài)功能磁共振數(shù)據(jù),計算標(biāo)準(zhǔn)化f ALLF值和Re Ho值,并進(jìn)行兩獨立樣本t檢驗(two-sample t-test)比較其差異。結(jié)果1.酒精依賴組f ALFF值明顯低于正常對照組(P0.001)的腦區(qū)包括:左側(cè)額中回、左側(cè)背外側(cè)額上回、左側(cè)中央前回、左側(cè)中央后回、左側(cè)中央旁小葉、左側(cè)楔前葉、左側(cè)頂上回及左側(cè)頂下緣角回、右側(cè)輔助運動區(qū)及右側(cè)額中回。酒精依賴組f ALFF值明顯高于正常對照組(P0.001)的腦區(qū)包括:右側(cè)眶部額下回、右側(cè)顳下回及右側(cè)島蓋部額下回。2.酒精依賴組Re Ho值明顯低于正常對照組(P0.001)的腦區(qū)包括:右側(cè)眶部額下回、右側(cè)眶部額中回、右側(cè)中央后回及左側(cè)眶部額葉內(nèi)側(cè)面。酒精依賴組Re Ho值明顯高于正常對照組(P0.001)的腦區(qū)包括:右側(cè)枕中回、右側(cè)顳下回。3.酒精依賴組行為學(xué)成功采集16人,正常對照組行為學(xué)成功采集19人。結(jié)果發(fā)現(xiàn),兩組對恐懼圖片和高興圖片的再認(rèn)擊中率明顯高于中性圖片,酒精依賴組與健康對照組對高興圖片的記憶效果無顯著差異性(P=0.10),對恐懼圖片、中性圖片和傷心圖片有顯著差異性(P均0.05)。結(jié)論1.酒精依賴者靜息態(tài)腦功能發(fā)生異常改變,主要累及到執(zhí)行控制、感覺運動及情緒記憶等相關(guān)腦區(qū),導(dǎo)致局部腦功能發(fā)生增強或減弱。2.酒精依賴者情緒記憶能力減退,對不同情緒圖片的記憶結(jié)果有顯著差異性,不同情緒圖片的情緒記憶增強效應(yīng)具有不同的調(diào)節(jié)機制。
[Abstract]:Objective to evaluate the changes of brain function and the characteristics of emotional memory in patients with alcohol dependence by resting f MRI and behavioral methods. Methods A total of 23 male alcohol-dependent subjects and 20 male healthy controls were included. The program was programmed with E-prime software and the behavioral data were collected from all subjects by associative notebook computer. The behavioral results were analyzed by single factor variance analysis (one-way ANOVA) and multiple comparative analysis. The resting fMRI data of the two groups were collected by 3.0T MR instrument, the normalized f ALLF and Re Ho values were calculated, and the difference was compared by two independent sample t-test (two-sample t-test). Result 1. In alcohol dependent group, the f ALFF values were significantly lower than those in the control group (P0. 001), including left middle frontal gyrus, left dorsolateral superior frontal gyrus, left precentral gyrus, left posterior central gyrus, left paracentral lobule, left precuneal lobe. Left superior parietal gyrus and left inferior parietal corner gyrus, right auxiliary motor area and right middle frontal gyrus. The value of f ALFF in alcohol dependent group was significantly higher than that in normal control group (P0. 001), including right inferior orbital gyrus, right infratemporal gyrus and right subfrontal gyrus of the right insular tegmental part. The Re Ho values of alcohol dependent group were significantly lower than that of normal control group (P0. 001), including right inferior orbitofrontal gyrus, right middle orbitofrontal gyrus, right posterior central gyrus and left medial orbital frontal lobe. The Re Ho values of alcohol dependent group were significantly higher than that of normal control group (P0. 001), including right middle occipital gyrus and right inferior temporal gyrus. The ethology of alcohol dependent group was successfully collected from 16 people, and that of normal control group was 19. The results showed that the recognition rate of fear picture and happy picture in the two groups was significantly higher than that in the neutral picture. There was no significant difference between the alcohol dependent group and the healthy control group in the memory effect of the happy picture (P0. 10). There was significant difference between neutral picture and sad picture (P 0.05). Conclusion 1. Abnormal changes of resting brain function occurred in alcohol-dependent patients, mainly involving executive control, sensorimotor, emotional memory and other related brain areas, resulting in the enhancement or weakening of local brain function. 2. The ability of emotional memory of alcohol-dependent patients was decreased, and the memory results of different emotional pictures were significantly different. The enhancement effect of emotional memory of different emotional pictures had different regulatory mechanisms.
