精神分裂癥患者攻擊行為的結(jié)構(gòu)磁共振研究

【摘要】:目的運用結(jié)構(gòu)磁共振方法研究伴有攻擊行為的精神分裂癥(schizophrenia,SZ)患者腦灰質(zhì)結(jié)構(gòu)特點,并探索其攻擊行為與腦灰質(zhì)結(jié)構(gòu)的關(guān)系。方法收集伴有攻擊行為的精神分裂癥患者18例(SZ1組),以及性別、年齡與之相匹配的不伴有攻擊行為精神分裂癥患者18例(SZ2組)和正常對照18名。采用Buss和Perry攻擊問卷(BussPerry aggression questionnaire,BP)中文版評估患者的攻擊行為,采用3.0 T磁共振對被試進(jìn)行腦結(jié)構(gòu)掃描,應(yīng)用基于體素的形態(tài)學(xué)測量方法對影像數(shù)據(jù)進(jìn)行分析處理。結(jié)果與對照組相比,精神分裂癥患者腦灰質(zhì)體積改變的區(qū)域主要位于雙側(cè)的額葉、顳葉和枕葉(P0.05,Alpha Sim校正);與SZ2組相比,SZ1組灰質(zhì)體積增加的腦區(qū)主要位于右側(cè)的緣上回、中央后回、雙側(cè)的島葉、眶額回(P0.05,Alpha Sim校正)。精神分裂癥患者的BP評分與其右側(cè)島葉、中央后回、緣上回等腦區(qū)的灰質(zhì)體積呈正關(guān)聯(lián)(P0.01,Alpha Sim校正)。結(jié)論精神分裂癥患者的攻擊行為與腦灰質(zhì)體積改變存在關(guān)聯(lián),中央后回、島葉和緣上回等腦區(qū)可能參與精神分裂癥患者攻擊行為的神經(jīng)機(jī)制。
[Abstract]:Objective to study the structural characteristics of gray matter in patients with aggressive schizophrenia (schizophrenia,SZ) by structural magnetic resonance (Mr), and to explore the relationship between aggressive behavior and gray matter structure. Methods 18 schizophrenic patients with aggressive behavior (SZ1 group), 18 patients with schizophrenia without aggressive behavior (SZ2 group) and 18 normal controls (gender and age matched) were collected. The Chinese version of Buss and Perry attack questionnaire (BussPerry aggression questionnaire,BP) was used to evaluate the aggressive behavior of the patients. The brain structure was scanned by 3.0T MRI and the image data were analyzed and processed by morphometry based on voxel. Results compared with the control group, the areas of gray matter volume changes in schizophrenic patients were mainly located in bilateral frontal lobe, temporal lobe and occipital lobe (P0.05 Alpha Sim correction), and the increased gray matter volume in SZ1 group was mainly located in the superior marginal gyrus of the right side compared with SZ2 group. Posterior central gyrus, bilateral insular lobe, orbital frontal gyrus (P0.05 Sim correction). The BP score was positively correlated with the gray matter volume in the right insular lobe, posterior central gyrus and superior marginal gyrus (P0.01 Alpha Sim correction). Conclusion the aggressive behavior of schizophrenic patients is associated with the change of gray matter volume. The posterior central gyrus, the insular lobe and the superior marginal gyrus may be involved in the neuromechanism of aggressive behavior in schizophrenic patients.
