【摘要】：目的探缺血性腦白質(zhì)病變與認(rèn)知障礙的關(guān)系,為血管性癡呆的預(yù)防、早期診斷和治療提供理論依據(jù)。方法選擇138例經(jīng)顱腦MRI證實(shí)的腦白質(zhì)病變患者,并分為缺血性腦白質(zhì)病變組和非缺血性腦白質(zhì)病變組,分別進(jìn)行簡(jiǎn)易智能狀態(tài)檢查量表(MMSE)評(píng)估比較,并對(duì)白質(zhì)病變進(jìn)行分級(jí),進(jìn)行組內(nèi)及兩組患者認(rèn)知功能?chē)?yán)重程度比較。結(jié)果缺血性白質(zhì)病變組認(rèn)知功能障礙發(fā)生率及Ⅱ、Ⅲ段白質(zhì)病變者的認(rèn)識(shí)功能障礙的嚴(yán)重程度明顯高于非缺血性白質(zhì)病變組;組內(nèi)比較缺血性白質(zhì)病變組和非缺血性白質(zhì)病變組認(rèn)知功能障礙與白質(zhì)病變嚴(yán)重分級(jí)呈正相關(guān),而二者無(wú)明顯關(guān)系。結(jié)論缺血性白質(zhì)病及其嚴(yán)重程度對(duì)認(rèn)知功能障礙的發(fā)生及嚴(yán)重程度有顯著影響,而非缺血性白質(zhì)病則對(duì)認(rèn)知功能障礙影響較小。
