【摘要】：研究目的 觀察小劑量西酞普蘭與小劑量多塞平治療廣泛性焦慮障礙的有效劑量、療效和臨床安全性,以及兩種藥物的臨床作用之間的差異性。 研究方法 將78例符合廣泛性焦慮障礙(GAD)診斷標(biāo)準(zhǔn)的門診患者,按就診的先后順序隨機(jī)分為兩組,分別口服西酞普蘭(20mg)和多塞平(12.5mg)連續(xù)治療12周,于治療前、治療4周、8周、12周后分別用漢密爾頓焦慮量表(HAMA)、精神性焦慮因子(F1)、軀體性焦慮因子(F2)、漢密爾頓抑郁量表(HAMD)對(duì)患者進(jìn)行評(píng)分,觀察不同治療時(shí)間兩組GAD患者HAMA、F1、F2、HAMD評(píng)分的變化,進(jìn)一步分析不同治療時(shí)間上兩組藥物對(duì)GAD患者的作用；在治療終點(diǎn),計(jì)算兩組藥物治療GAD患者關(guān)于HAMA的痊愈率、顯效率、有效率,對(duì)比分析兩組藥物對(duì)于GAD患者的療效差異性；此外,還觀察了治療過(guò)程中患者出現(xiàn)的不良反應(yīng)發(fā)生率,評(píng)估其臨床安全性。 研究結(jié)果 與治療前相比,兩組患者治療后HAMA、F1、F2、HAMD評(píng)分較治療前降低(P0.05),并隨著治療時(shí)間的延長(zhǎng)下降趨勢(shì)逐漸顯著(P0.01),組內(nèi)對(duì)照采用方差分析,差異均有統(tǒng)計(jì)學(xué)意義；在治療后12周時(shí),兩組藥物臨床總體療效及不良反應(yīng)發(fā)生率之間差異均無(wú)統(tǒng)計(jì)學(xué)意義。 研究結(jié)論 通過(guò)上述研究結(jié)果,我們發(fā)現(xiàn),通過(guò)兩組之間的總體療效評(píng)估結(jié)果,我們認(rèn)為小劑量西酞普蘭和小劑量多塞平治療廣泛性焦慮障礙均具有較好的作用,兩種藥物總體療效無(wú)明顯差異性,且兩種藥物的臨床安全性均較好。
[Abstract]:Objective to observe the efficacy, efficacy and safety of low dose citalopram and low dose doxepin in the treatment of generalized anxiety disorder. Methods Seventy-eight outpatients with generalized anxiety disorder (GAD) were randomly divided into two groups: oral citalopram (20mg) and doxepin (12.5mg) for 12 weeks. After 4 weeks, 8 weeks and 12 weeks of treatment, the patients were assessed with Hamilton anxiety scale (HAMA), (HAMA), mental anxiety factor (F1), somatic anxiety factor (F2) and Hamilton depression scale (HAMD) respectively. The changes of HAMA,F1,F2,HAMD score in two groups of GAD patients were observed. At the end of the treatment, the cure rate, effective rate and effective rate of HAMA in the patients with GAD were calculated, and the difference of the curative effect between the two groups for GAD patients was compared. In addition, the incidence of adverse reactions in the course of treatment was observed and its clinical safety was evaluated. Results compared with before treatment, the HAMA,F1,F2,HAMD score of the two groups was lower than that before treatment (P0.05), and with the prolongation of the treatment time, the trend of decrease was gradually significant (P0.01). The variance analysis was used in the control group, the difference was statistically significant. At 12 weeks after treatment, there was no significant difference in the overall efficacy and incidence of adverse reactions between the two groups. Conclusion based on the above results, we found that both low dose citalopram and low dose doxepin were effective in the treatment of generalized anxiety disorder. There was no significant difference in the overall efficacy of the two drugs, and the clinical safety of the two drugs was good.
