【摘要】：目的評價Lawton日常生活能力量表鑒別癡呆與輕度認知損害的能力。方法根據(jù)納入和排除標準篩選有記憶下降主訴的患者,結(jié)合臨床癡呆分級量表得分分為認知正常組、輕度認知損害組和癡呆組,采用1~4分多水平計分方法對14個日常生活能力量表(ADL)測試條目進行評估,采用受試者工作曲線對量表區(qū)分不同認知損害患者的能力進行評價。結(jié)果多水平ADL量表具有很好的內(nèi)部一致性(克朗巴赫α系數(shù)=0.93),ADL評分與MMSE呈負相關(guān)關(guān)系(偏相關(guān)系數(shù)為-0.793,P=0.000)。以16分作為分界值(≥16分為癡呆)區(qū)分癡呆與輕度認知損害最理想,敏感性和特異性分別為0.90、0.93,其中工具性日常生活能力評分(≥10分)具有相同的敏感性和特異性,但基本日常生活能力區(qū)分癡呆與輕度認知損害缺乏敏感性(P0.50)。結(jié)論多水平Lawton日常生活能力量表能有效地區(qū)分輕度認知損害和癡呆。
[Abstract]:Objective to evaluate the ability of Lawton ADL scale to distinguish dementia from mild cognitive impairment. Methods according to the inclusion and exclusion criteria, the patients with memory impairment were selected and divided into three groups: normal cognitive group, mild cognitive impairment group and dementia group. Fourteen ADL test items were evaluated by using a multi-level scoring method with 1-4 scores. The ability of the scale to distinguish patients with different cognitive impairment was evaluated by using the operating curve of the subjects. Results the multi-level ADL scale had good internal consistency (Cronbach 偽 coefficient 0.93) and the score of ADL was negatively correlated with MMSE (partial correlation coefficient was -0.793P 0.000).
