【摘要】：目的評估超聲(ultrasonography)在性別不安者(gender dysphoria)嗓音變調(diào)手術(shù)前后喉結(jié)構(gòu)測量方面的應(yīng)用價(jià)值。方法選取2016年8月至2017年4月就診于首都醫(yī)科大學(xué)附屬北京友誼醫(yī)院有改變音調(diào)意愿的性別不安者(男變女,male to female,MtF)5例,4例行支撐喉鏡下聲帶截短術(shù)(wendler glottoplasty)(其中1例同時(shí)行喉結(jié)成形術(shù)),1例行環(huán)甲接近術(shù)(cricothyroid approximation,CTA);所有患者術(shù)前、術(shù)后應(yīng)用超聲測量聲帶長度及環(huán)甲間隙距離,并于術(shù)后一個(gè)月測試嗓音基頻(F0)。結(jié)果超聲影像顯示患者的喉結(jié)構(gòu)及測量標(biāo)志清晰,除1例同時(shí)行喉結(jié)成形術(shù)的患者術(shù)后未行超聲檢查外,余3例行聲帶截短術(shù)的MtF患者術(shù)前與術(shù)后超聲測量聲帶長度差值(即聲帶截短長度)分別為0.37 cm(21%)、0.69 cm(37%)、0.40 cm(25%)。行環(huán)甲接近術(shù)的MtF患者術(shù)前超聲測量環(huán)甲間隙距離為1.0 cm,術(shù)后為-0.33 cm,環(huán)狀軟骨上移1.33 cm,而術(shù)前和術(shù)后CT測量環(huán)甲間隙分別為0.91和-0.30 cm,環(huán)狀軟骨上移1.2 cm,可見超聲測量與CT測量結(jié)果基本一致。除1例外,其余4例患者術(shù)后1個(gè)月嗓音F0均較術(shù)前明顯升高。結(jié)論喉結(jié)構(gòu)超聲測量可為性別不安者嗓音變調(diào)手術(shù)提供客觀喉部測量數(shù)據(jù),對術(shù)前與術(shù)后評估有重要應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the value of ultrasound (ultrasonography) in the measurement of laryngeal structure before and after the voice variant of gender dysphoria (gender dysphoria). Methods 5 cases of sexual unease (male to female, male to female, MtF) were selected from August 2016 to April 2017 in Beijing Friendship Hospital affiliated to Capital Medical University. The vocal cord truncation (Wendler glottoplasty) under the support laryngoscope (of which 1 cases were performed simultaneously with laryngoplasty) and 1 cases of ring nail approach (cricothyroid approximation, CTA); all patients were used to measure the length of the vocal cords and the distance of the ring nail clearance before operation, and the voice base frequency (F0) was tested one month after the operation. The results of ultrasonic imaging showed the patients. The structure of the larynx and the measurement marks were clear. In addition to 1 cases who had not performed sonography after larynx plasty, the remaining 3 cases of MtF patients with vocal cord truncation were 0.37 cm (21%), 0.69 cm (37%), 0.40 cm (25%), respectively, before and after the operation of the vocal cords. The distance of the circumferential space was 1 cm, -0.33 cm after operation and 1.33 cm of cricoid cartilage, while 0.91 and -0.30 cm, respectively, before and after operation, and 1.2 cm of the cricoid cartilage, respectively. The ultrasonic measurement was basically consistent with the results of CT measurement. Except 1 exceptions, the voice F0 of the other 4 patients was significantly higher than that before the operation. Conclusion larynx conclusion. Ultrasonic measurement can provide objective larynx measurement data for patients with gender imbalance, and is valuable for preoperative and postoperative evaluation.
