【摘要】：目的:本研究旨在探討伴和不伴污染/洗滌癥狀的強(qiáng)迫性障礙患者之間厭惡感的差異,為強(qiáng)迫性障礙的異質(zhì)性和個(gè)體化治療提供理論依據(jù)。方法:選取符合疾病和有關(guān)健康問題的國際統(tǒng)計(jì)分類第十次修訂本(ICD-10)中強(qiáng)迫性障礙診斷標(biāo)準(zhǔn)的患者66例,根據(jù)是否伴有污染/洗滌癥狀分為清洗組(n=26)及非清洗組(n=40),并選取年齡、性別、受教育程度匹配的正常對(duì)照51例。所有被試完成詞匯判斷任務(wù),記錄被試在核心厭惡詞、道德厭惡詞和中性詞厭惡判斷中的正確率、反應(yīng)時(shí)及厭惡程度評(píng)級(jí)結(jié)果,比較三組被試厭惡感的差異。結(jié)果:清洗組、非清洗組核心厭惡詞[(723±89)ms,(746±95)ms vs.(676±96)ms]及道德厭惡詞[(772±98)ms,(796±92)ms vs.(723±94)ms]的反應(yīng)時(shí)均高于正常對(duì)照組(均P0.01),且前兩組核心厭惡詞[(6.7±1.5),(6.9±1.6)vs.(5.8±1.7)]及道德厭惡詞[(6.8±1.7),(7.2±1.3)vs.(6.3±1.5)]的厭惡程度評(píng)級(jí)均高于正常對(duì)照組(均P0.05)。清洗組與非清洗組在兩類厭惡詞的正確率、反應(yīng)時(shí)及厭惡程度評(píng)級(jí)上差異均無統(tǒng)計(jì)學(xué)意義(均P0.05)。結(jié)論:強(qiáng)迫性障礙患者厭惡感均升高,伴和不伴污染/洗滌癥狀的患者之間厭惡感無差異。厭惡感升高可能與強(qiáng)迫性障礙的發(fā)生及發(fā)展均有關(guān),降低厭惡感可能是未來強(qiáng)迫性障礙的一個(gè)治療方向。
[Abstract]:Objective: to explore the difference of aversion between patients with and without pollution / washing symptoms and to provide theoretical basis for heterogeneity and individualized treatment of obsessive-compulsive disorder. Methods: a total of 66 patients with obsessive-compulsive disorder (OCD) in ICD-10 (International Statistical Classification of Diseases and related Health problems) were selected and divided into cleaning group (n = 26) and non-cleaning group (n = 40) according to whether they were accompanied with pollution / washing symptoms. Gender, education level matched normal control 51 cases. All subjects completed the task of lexical judgment, and recorded the correct rate, reaction time and aversion rating results of core disgust word, moral loathing word and neutral aversion word, and compared the difference of aversion feeling among the three groups. Results: the reaction time of core disgust words [723 鹵89 95)ms vs.(676 鹵96)ms] and moral disgust words (772 鹵98ms + 796 鹵92)ms vs.(723 鹵94)ms) in the cleaning group and non-cleaning group were higher than those in the normal control group (all P 0.01), and the scores of the core disgust words [6.7 鹵1.56.9 鹵1.6)vs.(5.8 鹵1.7] and the moral disgust words (6.8 鹵1.7 鹵1.3)vs.(6.3 鹵1.5) in the former two groups were higher than those in the normal control group (all P 0.05). There was no significant difference between the cleaning group and the non-cleaning group in the correct rate, reaction time and aversion degree of the two kinds of disgust words (all P 0.05). Conclusion: the aversion of obsessive-compulsive disorder patients is increased, and there is no difference between the patients with and without pollution / washing symptoms. The increase of aversion may be related to the occurrence and development of obsessive-compulsive disorder, and reducing aversion may be one of the therapeutic directions of obsessive-compulsive disorder in the future.
