【摘要】：目的探討石杉?jí)A甲治療對(duì)老年血管性癡呆(VD)患者血液流變學(xué)及P300電位的影響。方法 90例老年VD患者隨機(jī)分為觀察組與對(duì)照組各45例,對(duì)照組給予基礎(chǔ)治療,觀察組在對(duì)照組基礎(chǔ)上加用石杉?jí)A甲,對(duì)比兩組血液流變學(xué)改變及P300電位情況。結(jié)果與治療前比較觀察組治療后簡(jiǎn)易精神狀態(tài)量表(MMSE)評(píng)分顯著增高、臨床癡呆量表(CDR)評(píng)分與日常生活活動(dòng)能力(ADL)評(píng)分顯著降低(P0.05);對(duì)照組上述指標(biāo)治療前后無(wú)顯著差異(P0.05)。治療后觀察組MMSE、CDR、ADL評(píng)分顯著優(yōu)于對(duì)照組(P0.05)。觀察組治療前后血漿黏度、全血高切黏度、全血中切黏度、全血低切黏度差異有統(tǒng)計(jì)學(xué)意義(P0.05),血細(xì)胞比容無(wú)顯著差異(P0.05),上述指標(biāo)對(duì)照組治療前后無(wú)統(tǒng)計(jì)學(xué)差異(P0.05),觀察組治療后顯著低于對(duì)照組(P0.05);與治療前比較,治療后觀察組P300電位波幅顯著增加,潛伏期顯著縮短(P0.05),對(duì)照組無(wú)明顯變化(P0.05),觀察組治療后波幅顯著比對(duì)照組高,而潛伏期則顯著比對(duì)照組短(P0.05)。結(jié)論石杉?jí)A甲能夠有效改善VD患者微循環(huán)、增加腦血流量,從而提高老年VD患者的認(rèn)知功能、自理能力和智能,改善癡呆癥狀,P300電位可作為評(píng)估VD病程與療效監(jiān)測(cè)的重要指標(biāo)。
[Abstract]:Objective to investigate the effects of Huperzine A treatment on hemorheology and P300 potential in patients with vascular dementia (VD). Methods 90 elderly patients with VD were randomly divided into two groups: the observation group (n = 45) and the control group (n = 45). The control group was treated with basic therapy. The observation group was treated with Huperzine A on the basis of the control group. The changes of hemorheology and P300 potential were compared between the two groups. Results compared with those before treatment, the MMSE score of the observation group was significantly higher than that of the control group, and the scores of the clinical dementia scale and ADLL were significantly lower than those of the control group (P 0.05), but there was no significant difference before and after treatment in the control group. After treatment, the ADL score of MMSE CDRL in the observation group was significantly better than that in the control group (P 0.05). In the observation group, plasma viscosity, whole blood high shear viscosity and whole blood shear viscosity were observed before and after treatment. The difference of whole blood low shear viscosity was statistically significant (P 0.05), and there was no significant difference in blood cell volume (P 0.05). There was no significant difference between the above indexes before and after treatment in the control group (P 0.05), and the observation group was significantly lower than that in the control group (P 0.05) after treatment, and compared with the control group before and after treatment. After treatment, the amplitude of P300 potential in the observation group was significantly increased, and the latent period was shortened significantly (P 0.05), but no significant change was found in the control group (P 0.05). The amplitude of the wave in the observation group was significantly higher than that in the control group, but the latency was significantly shorter than that in the control group (P 0.05). Conclusion Huperzine A can effectively improve microcirculation and increase cerebral blood flow in VD patients, thus improving cognitive function, self-care ability and intelligence of senile VD patients. Improving P300 potential of dementia symptoms can be used as an important index to evaluate the course of VD and monitor curative effect.
