青少年首發(fā)抑郁癥患者大腦靜息狀態(tài)功能網(wǎng)絡(luò)小世界特性研究

【摘要】:目的利用復(fù)雜網(wǎng)絡(luò)理論比較青少年首發(fā)未用藥抑郁癥患者與正常對照組之間大腦小世界功能網(wǎng)絡(luò)拓?fù)浣Y(jié)構(gòu)的異同點,以期進一步闡明青少年抑郁癥患者的神經(jīng)病理發(fā)病機制。方法共納入研究對象32例,青少年抑郁癥患者組16例,符合美國精神疾病診斷與統(tǒng)計手冊第4版(DSM-IV)的首發(fā)單相青少年抑郁癥患者,以往從未接受過任何治療;正常對照組16名,兩組間年齡、教育程度相匹配。所有被試對象均為右利手。采用不同的相關(guān)系數(shù)作為閾值構(gòu)建大腦復(fù)雜網(wǎng)絡(luò)。結(jié)果青少年首發(fā)未服藥患者組大腦神經(jīng)網(wǎng)路拓?fù)浣Y(jié)構(gòu),較正常健康對照呈現(xiàn)一種相對無序混亂的狀態(tài)。青少年抑郁癥組大腦網(wǎng)絡(luò)小世界特性明顯下降,神經(jīng)網(wǎng)絡(luò)整體運行效率下降。杏仁核在大腦拓?fù)浣Y(jié)構(gòu)中度值上升,杏仁核與顳葉皮層、杏仁核與頂葉皮層、杏仁核與中央后回、前額葉與頂下小葉等腦區(qū)之間功能連接增高,并且功能連接改變與患者病程呈正相關(guān)。青少年抑郁癥組中,靜息態(tài)背景下,前扣帶回、前額葉背外側(cè)段、前額葉內(nèi)側(cè)段、前額葉下段、島葉和顳葉皮層度值增加。結(jié)論杏仁核在抑郁癥發(fā)生、發(fā)展中起重要的作用,有關(guān)杏仁核的神經(jīng)網(wǎng)絡(luò)拓?fù)浣Y(jié)構(gòu)異常可能是潛在的青少年抑郁癥患者抑郁易感性、疾病發(fā)展的生物學(xué)標(biāo)記。抑郁癥患者大腦多個腦區(qū)存在結(jié)構(gòu)和功能的異常。
[Abstract]:Objective to compare the topological structure of brain small-world functional networks between first-episode depression patients and normal controls by using complex network theory in order to elucidate the neuropathological pathogenesis of adolescent depression patients. Methods A total of 32 patients with adolescent depression and 16 patients with adolescent depression were enrolled in the study. The first episode of monophasic adolescent depression, which was in accordance with the fourth edition of DSM-IVof the American Handbook for the diagnosis and Statistics of Mental Disorders, had never received any treatment in the past, while 16 patients in the normal control group were enrolled in the study. The age and educational level of the two groups were matched. All subjects were right-handed. Different correlation coefficients are used as thresholds to construct complex brain networks. Results the topological structure of neural network in the group of first-episode patients without medication showed a relatively disordered state compared with the normal control group. In adolescent depression group, the characteristics of small world of brain network decreased significantly, and the overall operating efficiency of neural network decreased. The middle value of amygdala increased in the brain topology, and the functional connections between amygdala and temporal cortex, amygdala and parietal cortex, amygdala and posterior central gyrus, prefrontal lobe and subparietal lobule were increased. And the change of functional connection was positively correlated with the course of disease. In the adolescent depression group, the cortical degree of anterior cingulate gyrus, dorsolateral prefrontal lobe, medial prefrontal lobe, lower prefrontal lobe, island lobe and temporal lobe increased. Conclusion amygdala plays an important role in the occurrence and development of depression. The abnormal neural network topology of amygdala may be a biological marker of depression susceptibility and disease development in adolescent depression patients. There are structural and functional abnormalities in multiple regions of the brain in patients with depression.
