【摘要】：背景腎功能與認(rèn)知功能之間的關(guān)系尚存爭(zhēng)議,影響老年人認(rèn)知功能障礙(CI)的因素眾多,血肌酐(Scr)與老年人CI的相關(guān)性鮮見(jiàn)報(bào)道。目的探討同等腎功能下老年人CI與Scr水平的關(guān)系,以期為老年人CI的臨床治療提供理論基礎(chǔ)和診療依據(jù)。方法 2014~(-1)1~(-1)3至2014~(-1)2-21采用分層隨機(jī)抽樣法在江蘇省如皋市江安鎮(zhèn)31個(gè)村選取符合納入標(biāo)準(zhǔn)的1 757例老年人為研究對(duì)象。收集受試者一般資料;采用改良長(zhǎng)谷川癡呆量表(HDS-R)進(jìn)行認(rèn)知功能評(píng)價(jià),按照HDS-R總分,21.5分為無(wú)認(rèn)知功能障礙(NCI),≤21.5分為CI;記錄實(shí)驗(yàn)室檢查結(jié)果[總膽固醇(TC)、三酰甘油(TG)、低密度脂蛋白膽固醇(LDL-C)、高密度脂蛋白膽固醇(HDL-C)、Scr水平及估算腎小球?yàn)V過(guò)率(e GFR)],根據(jù)e GFR對(duì)受試者進(jìn)行腎功能分級(jí):Ⅰ級(jí),腎功能正常,e GFR≥90 ml·min~(-1)·(1.73m~2)~(-1);Ⅱ級(jí),腎功能輕度損傷,60 ml·min~(-1)·(1.73 m~2)~(-1)≤e GFR90 ml·min~(-1)·(1.73 m~2)~(-1);≥Ⅲ級(jí),腎功能中重度損傷,e GFR60 ml·min~(-1)·(1.73 m~2)~(-1)。結(jié)果 1 757例受試者中,男820例,女937例;平均年齡(75.3±3.9)歲;NCI者823例,CI者934例;腎功能分級(jí):Ⅰ級(jí)275例(NCI者154例,CI者121例);Ⅱ級(jí)1 001例(NCI者513例,CI者488例);≥Ⅲ級(jí)481例(NCI者156例,CI者325例)。Ⅰ級(jí)腎功能下,NCI者男性率、Scr水平高于CI者,腦卒中發(fā)生率、冠心病發(fā)生率、TG水平低于CI者(P0.05)。Ⅱ級(jí)腎功能下,NCI者男性率、Scr水平高于CI者,年齡小于CI者,TG水平低于CI者(P0.05)�！茛蠹�(jí)腎功能下,NCI者男性率、Scr水平高于CI者,年齡小于CI者(P0.05)。Ⅰ級(jí)腎功能下,HDS-R總分與Scr水平呈正相關(guān)(r=0.506,P0.001);Ⅱ級(jí)腎功能下,HDS-R總分與Scr水平呈正相關(guān)(r=0.444,P0.001);≥Ⅲ級(jí)腎功能下,HDS-R總分與Scr水平呈正相關(guān)(r=0.233,P0.001)。結(jié)論同等腎功能下,高水平Scr對(duì)老年人CI具有一定的保護(hù)作用,且在腎功能良好情況下作用效果更顯著。
