【摘要】：目的系統(tǒng)評價氯氮平片聯(lián)合利培酮片治療難治性精神分裂癥的安全性。方法電子檢索Medline、Em Base、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫等中英文數(shù)據(jù)庫,截至2016-11-30。納入關(guān)于氯氮平合并利培酮治療難治性精神分裂癥的隨機(jī)對照試驗(yàn)(RCT)、半RCT(Quasi-RCT),試驗(yàn)組予以氯氮平聯(lián)合利培酮治療,對照組予氯氮平治療。依據(jù)Cochrane 5.1.0版質(zhì)量評價標(biāo)準(zhǔn)評價研究質(zhì)量和提取數(shù)據(jù)進(jìn)行Meta分析。結(jié)果共納入9個研究,共計781例患者。Meta分析結(jié)果顯示:氯氮平聯(lián)合利培酮治療難治性精神分裂癥的安全性較高,在便秘、流涎、嗜睡、頭暈、白細(xì)胞減少、心動過速、心電圖異常等7個方面的藥物不良反應(yīng)發(fā)生率合用組均低于單用組(P0.05),在肌張力增高、靜坐不能、震顫等3個藥物不良反應(yīng)方面組間差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論氯氮平聯(lián)合利培酮治療難治性精神分裂癥不是簡單的藥物疊加,可減少單用時因劑量過高導(dǎo)致的藥物不良反應(yīng),安全性較高,可作為難治性分裂癥治療的策略之一。
[Abstract]:Objective to evaluate the safety of clozapine combined with risperidone in the treatment of refractory schizophrenia. Methods to search the Chinese and English databases such as Medlinesite Em Base, Chinese Biomedical Literature Database and so on, as of June 11, 30, 2016. The randomized controlled trial of clozapine combined with risperidone in the treatment of refractory schizophrenia was conducted in RCTX and semi-RCTA Quasi-RCTA. The experimental group was treated with clozapine combined with risperidone and the control group was treated with clozapine. According to the quality evaluation standard of Cochrane 5.1.0, the research quality was evaluated and the extracted data were analyzed by Meta. Results A total of 781 patients. Meta-analysis showed that clozapine combined with risperidone was more safe in constipation, salivation, drowsiness, dizziness, leukopenia, tachycardia. The incidence of adverse drug reactions in the combination group was lower than that in the single use group (P 0.05). There was no significant difference among the three groups in terms of muscle tension, sedentary inability, tremor and other adverse drug reactions. Conclusion clozapine combined with risperidone is not a simple drug superposition for refractory schizophrenia, it can reduce the adverse drug reaction caused by excessive dosage in single use, and it is safe and can be used as one of the strategies for the treatment of refractory schizophrenia.
