ADHD癥狀及功能損害評(píng)定量表的初步編制與信效度分析

【摘要】:目的:結(jié)合目前臨床需要,初步編制ADHD癥狀及功能損害評(píng)定量表,評(píng)價(jià)量表的心理測(cè)量學(xué)特征。方法:(1)在既往研究基礎(chǔ)上,提出量表編制的理論結(jié)構(gòu),包括臨床癥狀和功能損害兩個(gè)部分。(2)通過(guò)文獻(xiàn)回顧、訪談和專家論證確定理論結(jié)構(gòu)并建立初步條目庫(kù)。(3)通過(guò)預(yù)調(diào)查篩選條目形成68個(gè)條目的專家問(wèn)卷。(4)通過(guò)Delphi法進(jìn)一步修改、合并,形成59個(gè)條目的量表初稿。(5)在412名ADHD患兒和322名正常兒童中進(jìn)行量表初稿的信效度研究:通過(guò)項(xiàng)目分析法進(jìn)一步篩選條目,探索性因子分析和驗(yàn)證性因子分析確定量表因子結(jié)構(gòu),形成含44個(gè)條目、6個(gè)因子的量表修改稿。相關(guān)分析驗(yàn)證量表的效標(biāo)效度,內(nèi)部一致性、重測(cè)信度檢驗(yàn)量表信度,并檢驗(yàn)量表反應(yīng)度。結(jié)果:(1)完成ADHD癥狀及功能損害評(píng)定量表的初步編制,該量表適用于6-12歲學(xué)齡ADHD兒童,為父母評(píng)價(jià),Likert5級(jí)計(jì)分,含44個(gè)條目,分為6個(gè)因子:多動(dòng)沖動(dòng)(8條)、自我控制(4條)、注意缺陷(9條)、自我管理(7條)、學(xué)業(yè)表現(xiàn)(7條)和社會(huì)交往(9條),將6個(gè)因子分為臨床癥狀、功能損害兩個(gè)分量表。(2)量表具有良好的效度,6因子模型擬合良好,各因子與相應(yīng)效標(biāo)均呈顯著相關(guān),各因子與總分之間均呈顯著相關(guān)(r=0.690-0.841)。(3)量表具有良好的信度,各因子Cronbach’sα系數(shù)=0.822-0.937,全量表Cronbach’sα系數(shù)=0.976,各因子間隔6周重測(cè)信度r=0.562-0.779,總分r=0.816(P均0.01)。(4)量表能靈敏反映患兒在治療前后癥狀和功能的改善情況。診斷敏感性=0.842,特異性=0.901,能正確區(qū)分70.7%患兒的臨床亞型和53.8%患兒的班級(jí)排名。結(jié)論:ADHD癥狀及功能損害評(píng)定量表能夠有效評(píng)估6-12歲ADHD兒童的癥狀和功能損害嚴(yán)重程度,具有良好的信效度,適合在臨床上用于探索ADHD的癥狀及功能受損評(píng)估、輔助診斷、治療效果和預(yù)后評(píng)價(jià)。
[Abstract]:Objective: according to the current clinical needs, preliminary preparation of ADHD symptoms and functional damage assessment, evaluation of the psychological measurement characteristics. Methods: (1) based on previous study, put forward the theory of structure scale, two parts including clinical symptoms and functional damage. (2) through the literature review, interview and expert argumentation theory to determine the structure and establish the preliminary items. (3) through the pre survey form a 68 item screening questionnaires. (4) further modified by Delphi method with 59 entries, the formation of the draft scale. (5) to study on reliability and validity of the draft scale in 412 children with ADHD and 322 normal children, through the project analysis further screening items, determine the scale factor structure of exploratory factor analysis and confirmatory factor analysis, containing 44 entries, 6 factor scale revised.
