【摘要】：目的評價老年人快速認(rèn)知篩查測驗(yàn)識別輕度認(rèn)知功能障礙(MCI)和早期癡呆(AD)的效能。方法 60歲及以上的輕度AD患者84例、MCI患者112例、正常老人(NC)196例,采用老年人快速認(rèn)知篩查測驗(yàn)(QCST-E)、簡易精神狀態(tài)檢查量表(MMSE)、日常生活能力(ADL)量表、臨床癡呆評定量表(CDR)、華文認(rèn)知能力量表(CCAS)進(jìn)行認(rèn)知功能評估。QCST-E分別測量12個認(rèn)知領(lǐng)域:即刻記憶、實(shí)物命名、視覺空間、言語流暢性、數(shù)字廣度、抽象能力、聽覺模仿、視覺模仿、動作指令、延遲記憶、簡單計算、時空定向,最高總分83分,得分越低,認(rèn)知功能損害越嚴(yán)重。結(jié)果輕度AD組、MCI組、NC組QCST-E總分分別為(51.47±3.91)分、(61.47±2.96)分、(72.60±3.01)分,差異顯著(P0.01)。QCST-E各領(lǐng)域和總分與年齡、受教育年限及MMSE、ADL、CCAS得分顯著相關(guān)(均P0.01)。根據(jù)不同文化程度制定QCST-E的劃界分,其中QCST-E總分識別MCI與正常老人的總體敏感度為89.6%、特異度為94.5%,曲線下面積(AUC)為0.903(95%CI:0.857~0.963);識別MCI與輕度AD的總體敏感度為81.7%、特異度為88.6%,AUC值為0.898(95%CI:0.845~0.940)。結(jié)論老年人快速認(rèn)知篩查測驗(yàn)內(nèi)容全面、簡便易行,識別MCI的敏感度和特異度良好。
[Abstract]:Objective to evaluate the efficacy of rapid cognitive screening test in the identification of mild cognitive dysfunction (MCI) and early dementia (AD) in the elderly. QCST-EN, MMSE and ADL were used in the rapid cognitive screening test of the elderly. QCST-E was used to measure 12 cognitive domains: immediate memory, physical naming, visual space, speech fluency, number span, abstract ability, auditory imitation. Visual imitation, action instruction, delayed memory, simple calculation, space-time orientation, maximum total score of 83 points, the lower the score, the more serious the cognitive impairment. Results the total score of QCST-E in the NC group of mild AD group was 51.47 鹵3.91) and the score was 61.47 鹵2.96) and 72.60 鹵3.01). The difference was significant (P 0.01). QCST-E scores were significantly correlated with age, years of education and scores of QCST-E (all P 0.01). The demarcation scores of QCST-E were determined according to different education levels. The overall sensitivity, specificity and area under the curve of QCST-E in identifying MCI and normal elderly were 89.6, 94.5and 0.903 / 95CI: 0.857 / 0.963respectively, and the overall sensitivity and specificity of identifying MCI and mild AD were 81.7 and 88.66%, respectively. Conclusion the content of rapid cognitive screening test for the elderly is comprehensive, and the total sensitivity is 81.7% and the specificity is 88.66%.
