重慶市社會(huì)福利院老年人群阿爾茲海默癥發(fā)病及與維生素A營(yíng)養(yǎng)關(guān)系的調(diào)查研究

【摘要】:目的 通過(guò)調(diào)查重慶市社會(huì)福利院老年人群認(rèn)知功能現(xiàn)狀、AD發(fā)病情況以及維生素A營(yíng)養(yǎng)狀況,分析兩者之間的關(guān)系,從而探討VA營(yíng)養(yǎng)狀況與AD發(fā)病可能存在的聯(lián)系以及影響。 對(duì)象與方法 采用整群隨機(jī)抽樣的方法,在重慶市十二家公立及私立社會(huì)福利院抽取合格志愿受試者,并進(jìn)行現(xiàn)場(chǎng)問(wèn)卷調(diào)查及認(rèn)知功能測(cè)試、體格檢查和血清VA水平檢測(cè)。認(rèn)知功能測(cè)試采用HIS、HRSD、MMSE、 ADLIADL、ADAS、CDR量表,血清VA水平檢測(cè)采用高效液相色譜法(HPLC)。共收集到有效樣本322例。并按照AD和VA的評(píng)價(jià)標(biāo)準(zhǔn),將有效樣本分為認(rèn)知功能正常、AD輕度癡呆、AD中度癡呆組和VAN、MVAD、VAD組,分析不同認(rèn)知功能層面下血清VA水平的情況以及不同VA營(yíng)養(yǎng)狀況中認(rèn)知功能損害及其程度的差異。 結(jié)果 1.社會(huì)福利院老年人群女性年齡(79.59±7.52)比男性(75.08±8.82)偏大約5歲(P0.01),認(rèn)知功能損害人群所占比例較大(55.9%);認(rèn)知功能量表評(píng)分高年齡層(85~95歲)均低于低年齡層(65~75歲或57~85歲),其中ADL、IADL、ADAS和CDR統(tǒng)計(jì)學(xué)差異明顯(P0.05)。 2.社會(huì)福利院老年人群VAD(10.9%)和MVAD (30.1%)檢出率高,VAN的構(gòu)成比在認(rèn)知功能正常組明顯高于認(rèn)知功能損害組,MVAD和VAD的檢出率隨認(rèn)知功能損害加重而明顯上升(P0.05);且輕度癡呆組(1.07±0.38μmol/L)和中度癡呆組(1.01±0.34μmol/L)的血清VA水平均顯著低于認(rèn)知正常組(1.33±0.37μmol/L)(P0.05)。 MMSE、IADL、ADL、ADAS、CDR五個(gè)認(rèn)知功能量表的評(píng)分所反應(yīng)的認(rèn)知功能情況在VA正常組均要明顯好于VA缺乏組(包括MVAD和VAD),且有統(tǒng)計(jì)學(xué)差異(P0.05)。血清VA水平和文化程度與MMSE、IADL評(píng)分存在正相關(guān)(P0.01),與ADL、ADAS、CDR評(píng)分存在負(fù)相關(guān)(P0.05);年齡與ADL、ADAS和CDR呈正相關(guān)(P0.05)。 結(jié)論 重慶市社會(huì)福利院老年人群中AD認(rèn)知功能損害以及VA缺乏的情況普遍存在,認(rèn)知損害與文化程度和年齡存在相關(guān)性,年齡越大,文化程度越低,損害越容易發(fā)生。而老年人群VA營(yíng)養(yǎng)與認(rèn)知功能狀況之間也存在一定的相關(guān)性,低血清VA水平可能導(dǎo)致認(rèn)知功能的損害,以及認(rèn)知功能損害程度的加重,從而增加AD發(fā)病的風(fēng)險(xiǎn)。
[Abstract]:Purpose. By investigating the prevalence of AD and vitamin A nutrition in the elderly population in Chongqing Social Welfare Institute, the relationship between VA nutritional status and AD was analyzed, and the possible relationship between VA nutritional status and AD was discussed. Objects and methods. By cluster random sampling, qualified volunteers were selected from 12 public and private social welfare institutions in Chongqing, and the questionnaire survey and cognitive function test were carried out. Physical examination and serum VA level. Cognitive function test was performed with HIS-HRSDMMSE, ADLIADLLUADAS-CDR scale, and serum VA level was measured by HPLC. 322 effective samples were collected and assessed according to AD and VA evaluation criteria. The effective samples were divided into two groups: mild dementia with normal cognitive function and moderate dementia with mild AD and VANG MVADVAD. The levels of serum VA in different cognitive function levels and the differences of cognitive impairment and their degrees in different VA nutritional status were analyzed. Results. 1. The age of elderly women (79.59 鹵7.52) was higher than that of men (75.08 鹵8.82), and the proportion of people with cognitive impairment was 55.9%, and the score of cognitive function scale was 85 ~ 95 years old, which was lower than 6575 or 5785 years of age in the lower age group (P < 0.05). ADAS and CDR were significantly different in ADAS and CDR (P 0.05). 2. The detection rate of VAD-10.9 and MVAD 30.1in the elderly in social welfare institutions was significantly higher in the normal cognitive group than in the cognitive impairment group, and the detection rate of MVAD and VAD in the mild dementia group was significantly higher than that in the mild dementia group. The serum VA levels of 1.07 鹵0.38 渭 mol / L) and 1.01 鹵0.34 渭 mol / L of moderate dementia group were significantly lower than those of normal cognitive control group (1.33 鹵0.37 渭 mol / L, P 0.05). There was a positive correlation between serum VA level and MMSE IADL score (P 0.01), a negative correlation between serum VA level and CDR score (P 0.05), and a positive correlation between age and ADL ADAS and CDR (P 0.05). Conclusion. The AD cognitive impairment and VA deficiency were common in the elderly population of Chongqing Social Welfare Institute. The cognitive impairment was correlated with the education level and age. The older the age, the lower the education level. However, there is a certain correlation between VA nutrition and cognitive function in the elderly. Low serum VA level may lead to cognitive impairment and increase the degree of cognitive impairment. Thus increasing the risk of AD.
