硫氧還蛋白相互作用蛋白-NLRP3炎性小體通路在肝衰竭中作用與機制

背景:肝功能衰竭是由病毒、藥物、酒精等多種因素所致的一種臨床綜合征,主要表現(xiàn)為肝臟合成、解毒、排泄和生物轉(zhuǎn)化功能等發(fā)生嚴(yán)重障礙或失代償,臨床特征為凝血功能障礙、黃疸、肝性腦病、腹水等,病情重、發(fā)展快、病死率高。流行病學(xué)調(diào)查顯示,我國肝衰竭的主要病因為HBV感染,臨床上主要表現(xiàn)為慢加急性肝衰竭(acute on chronic liver failure,ACLF),且其發(fā)病率呈增加趨勢。肝衰竭的發(fā)病機制尚未完全闡明,是目前臨床與基礎(chǔ)研究的熱點。近年來NLRP3炎性小體在各種肝臟疾病中的作用受到了越來越多的關(guān)注,其結(jié)構(gòu)包括NOD樣受體NLRP3,銜接分子ASC及效應(yīng)分子pro-Caspase-1。當(dāng)NLRP3炎癥小體與銜接分子ASC形成復(fù)合物時,pro-Caspase-1被激活為成熟的Caspase-1,并最終導(dǎo)致IL-18及IL-1β的活化與釋放。目前已有研究證實,NLRP3炎癥小體的激活在對乙酰氨基酚所致急性肝損傷中發(fā)揮了免疫調(diào)節(jié)作用,但關(guān)于NLRP3炎性小體在肝衰竭發(fā)病過程中發(fā)揮的作用及其具體調(diào)節(jié)機制還缺乏足夠研究。NLRP3炎性小體的激活分為兩個步驟:首先,包含脂多糖在內(nèi)的多種細(xì)...
