【摘要】：目的探討腦白質(zhì)病變(WML)與急性腦梗死(ACI)患者近期預(yù)后的關(guān)系。方法選擇合并WML的ACI患者40例(WML組),同期未合并WML的ACI患者40例(對(duì)照組)。收集所有患者病歷資料(一般資料、入院時(shí)實(shí)驗(yàn)室檢查結(jié)果),采用改良Rankin預(yù)后等級(jí)量表評(píng)分(MRS)評(píng)價(jià)患者近期(出院1個(gè)月)預(yù)后;根據(jù)MRS將患者分為預(yù)后良好組46例、預(yù)后不良組34例;用受試者工作特征(ROC)曲線評(píng)價(jià)WML對(duì)ACI預(yù)后評(píng)價(jià)的價(jià)值;采用二元Logistic回歸分析WML與ACI患者近期預(yù)后的關(guān)系。結(jié)果與對(duì)照組比較,WML組年齡、MRS評(píng)分高(P均0.05);與預(yù)后良好組比較,預(yù)后不良組年齡、WML患病率、入院時(shí)美國(guó)國(guó)立衛(wèi)生院神經(jīng)功能缺損量表(NIHSS)評(píng)分高(P均0.05);年齡、WML、NIHSS評(píng)分評(píng)估ACI患者近期預(yù)后的ROC曲線下面積分別為0.71、0.64、0.74;經(jīng)二元Logistic回歸分析發(fā)現(xiàn),WML是ACI患者近期預(yù)后的危險(xiǎn)因素(OR=2.65,P0.05)。結(jié)論合并WML的ACI患者近期預(yù)后不良。
[Abstract]:Objective to investigate the relationship between (WML) and short-term prognosis of patients with acute cerebral infarction (ACI). Methods 40 patients with ACI complicated with WML (WML group) and 40 patients with ACI without WML at the same time (control group) were selected. The clinical data of all patients (general data, laboratory results at admission) were collected, and the short-term prognosis of patients was evaluated by modified Rankin prognosis rating scale (MRS). According to MRS, the patients were divided into good prognosis group (46 cases) and poor prognosis group (34 cases). The value of WML in evaluating the prognosis of ACI was evaluated by (ROC) curve of subjects' working characteristics. The relationship between WML and short-term prognosis of ACI patients was analyzed by binary Logistic regression. Results compared with the control group, the age and MRS score of the WML group were higher than those of the control group, and the age of the poor prognosis group, the prevalence of WML and the (NIHSS) score of the neurologic deficit scale of the National Hospital of Health at admission were higher than those of the control group, and the area under the ROC curve of age and WML,NIHSS score for evaluating the short-term prognosis of ACI patients were 0.71, 0.64 and 0.74, respectively. Binary Logistic regression analysis showed that WML was a risk factor for short-term prognosis of ACI patients (OR=2.65,P0.05). Conclusion the short-term prognosis of ACI patients with WML is poor.
