替格瑞洛在急性前壁心肌梗死急診經(jīng)皮冠狀動脈介入治療中的應(yīng)用

【摘要】：目的觀察替格瑞洛在急性前壁心肌梗死患者急診經(jīng)皮冠狀動脈介入治療(PCI)中的抗血小板效果。方法將67例急性前壁心肌梗死行急診PCI的患者隨機分為替格瑞洛組和氯吡格雷組。兩組入院后均接受其他常規(guī)治療,氯吡格雷組術(shù)前給予氯吡格雷負荷劑量、術(shù)后給予氯吡格雷標準劑量口服治療,替格瑞洛組術(shù)前給予替格瑞洛負荷劑量、術(shù)后給予替格瑞洛標準劑量口服治療。比較兩組血小板最大聚集率(MPAR)和血小板聚集抑制率(IPA)、手術(shù)前后TIMI血流分級及心臟彩超結(jié)果[左心室射血分數(shù)(LVEF)、左心室舒張末期內(nèi)徑(LVEDD)]、術(shù)后6個月主要不良心血管事件及出血并發(fā)癥。結(jié)果替格瑞洛組服藥后1、2、24 h及服藥后7 d MPAR低于氯吡格雷組,IPA高于氯吡格雷組(P均0.05);替格瑞洛組術(shù)后TIMI分級和LVEF高于氯吡格雷組,LVEDD低于氯吡格雷組(P均0.05)。替格瑞洛組主要不良心血管事件發(fā)生率低于氯吡格雷組(P0.05),兩組出血并發(fā)癥發(fā)生率無統(tǒng)計學差異。結(jié)論急診PCI治療急性前壁心肌梗死時,使用替格瑞洛抗血小板治療效果較氯吡格雷更佳,安全性更高。
[Abstract]:Objective to observe the antiplatelet effect of tigrilol in emergency percutaneous coronary intervention (PCI) in patients with acute anterior myocardial infarction (ACMI). Methods 67 patients with acute anterior myocardial infarction undergoing emergency PCI were randomly divided into tegrilol group and clopidogrel group. Both groups received other routine treatment after admission. Clopidogrel group was given clopidogrel load dose before operation, clopidogrel standard dose after operation, tegrilol load dose before operation and tegrilol standard dose after operation. The maximum platelet aggregation rate (MPAR) and platelet aggregation inhibition rate (IPA),) before and after operation were compared between the two groups. TIMI blood flow classification and cardiac color Doppler ultrasound results [left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD)], major adverse cardiovascular events and bleeding complications 6 months after operation. Results the MPAR of tigrilol group was lower than that of clopidogrel group at 1 h, 2 h, 24 h and 7 d after treatment, IPA was higher than that of clopidogrel group, TIMI grade and LVEF of tigrilol group were higher than those of clopidogrel group, and LVEDD was lower than that of clopidogrel group. The incidence of main adverse cardiovascular events in tigrilol group was lower than that in clopidogrel group (P 0.05). There was no significant difference in the incidence of bleeding complications between the two groups. Conclusion in the treatment of acute anterior myocardial infarction with emergency PCI, tigrilol is more effective and safe than clopidogrel in the treatment of acute anterior myocardial infarction.
