【摘要】：目的探討血糖水平對重組組織型纖維蛋白酶原激活劑(rt-PA)靜脈溶栓治療急性缺血性腦卒中預(yù)后的影響。方法選擇2014年5月至2015年7月在該院接受靜脈rt-PA溶栓治療的118例急性缺血性腦卒中患者。根據(jù)溶栓前血糖水平分為血糖正常組(血糖7.8 mmol/L,n=49),血糖偏高組(7.8~11.1 mmol/L,n=42),高血糖組(血糖11.1 mmol/L,n=27)。采用Rankin量表(mRS)評價(jià)神經(jīng)功能恢復(fù)情況,根據(jù)mRS評分分為預(yù)后良好組(n=71)和預(yù)后不良組(n=47)。分析影響rt-PA靜脈溶栓治療急性缺血性腦卒中預(yù)后的因素,比較不同血糖水平患者的預(yù)后。結(jié)果(1)多因素Logistic回歸分析顯示,影響rt-PA預(yù)后的危險(xiǎn)因素有:溶栓前血糖、溶栓前美國國立衛(wèi)生研究院卒中量表(NIHSS)評分、伴有糖尿病、溶栓后2 h及24 h收縮壓。(2)血糖偏高組和高血糖組顱內(nèi)出血發(fā)生率、癥狀性顱內(nèi)出血發(fā)生率、病死率、預(yù)后不良發(fā)生率明顯高于血糖正常組(P0.05)。結(jié)論溶栓前血糖≥7.8 mmol/L的急性缺血性腦卒中患者rt-PA靜脈溶栓治療預(yù)后較差,血糖水平是影響靜脈rt-PA溶栓治療預(yù)后的獨(dú)立因素。
