【摘要】：目的探討聽覺統(tǒng)合訓(xùn)練對(duì)額葉顱腦損傷患者記憶功能、認(rèn)知功能和血清腦源性神經(jīng)營(yíng)養(yǎng)因子(BDNF)的影響。方法采用數(shù)字隨機(jī)法將40例額葉顱腦損傷患者分為對(duì)照組(20例)和聽覺統(tǒng)合訓(xùn)練組(20例)。對(duì)照組給予常規(guī)康復(fù)治療;聽覺統(tǒng)合訓(xùn)練組給予聽覺統(tǒng)合訓(xùn)練,1次/d,5次/w。治療前后,分別采用修訂的韋氏記憶量表(WMS-CR)評(píng)測(cè)工作記憶,威斯康星評(píng)測(cè)系統(tǒng)(WCST)評(píng)測(cè)認(rèn)知功能,酶聯(lián)免疫吸附試驗(yàn)檢測(cè)血清BDNF的濃度。結(jié)果與對(duì)照組比較,視聽感覺統(tǒng)合訓(xùn)練組WMS-CR評(píng)測(cè)中的理解、背數(shù)和積累分測(cè)驗(yàn)得分顯著增高;WCST評(píng)測(cè)的完成分類數(shù)、正確應(yīng)答數(shù)、概念化水平百分?jǐn)?shù)、非持續(xù)性錯(cuò)誤數(shù)項(xiàng)評(píng)分優(yōu)于對(duì)照組,但總應(yīng)答數(shù)、錯(cuò)誤應(yīng)答數(shù)、持續(xù)性應(yīng)答數(shù)、持續(xù)性錯(cuò)誤數(shù)差異無統(tǒng)計(jì)學(xué)意義(P0.05);BDNF血清濃度較對(duì)照組升高(P0.05)。結(jié)論聽覺統(tǒng)合訓(xùn)練可改善額葉顱腦損傷患者的記憶功能、認(rèn)知功能,提高BDNF水平。
[Abstract]:Objective to investigate the effects of auditory integrative training on memory function, cognitive function and serum brain-derived neurotrophic factor (BDNF) in patients with frontal lobe craniocerebral injury. Methods 40 patients with frontal lobe craniocerebral injury were randomly divided into control group (20 cases) and auditory integrated training group (20 cases). The control group received routine rehabilitation and the auditory integrative training group received auditory integrative training once a day for 5 times per week. Before and after treatment, working memory was evaluated by revised Wechsler memory scale (WMS-CR), cognitive function was assessed by Wisconsin (WCST), and serum BDNF concentration was measured by Elisa. Results compared with the control group, the scores of comprehension, back count and accumulation subtest in the audiovisual sensory integration training group were significantly higher than those in the control group. The scores of complete classification, correct response, conceptualization level, non-persistent error number, WCST evaluation were better than those of the control group, but the total number of responses, the number of error responses, and the number of persistent responses were better than those of the control group. There was no significant difference in the number of persistent errors (P0.05). The serum concentration of BDNF was higher than that of control group (P0.05). Conclusion auditory integrative training can improve the memory function, cognitive function and BDNF level of patients with frontal lobe craniocerebral injury.
