【摘要】：目的探討胸痛中心的建立對(duì)急性ST段抬高型心肌梗死(STEMI)患者救治的影響。方法參照國(guó)際胸痛中心協(xié)會(huì)的要求建立胸痛中心及相應(yīng)的管理制度和救治流程。選取沈陽(yáng)軍區(qū)總醫(yī)院2015年5月-2016年3月收治的急性ST段抬高型心肌梗死(STEMI)患者為觀察組,未建立胸痛中心之前2014年1-12月收治的STEMI患者為對(duì)照組,共入選患者1088例,觀察組576例,對(duì)照組512例。采用回顧性非同期隊(duì)列研究方法,觀察組入院后進(jìn)入胸痛中心,對(duì)照組在胸痛中心成立前入院進(jìn)行常規(guī)救治。觀察兩組STEMI患者的一般情況、入院后首份心電圖完成時(shí)間以及10min內(nèi)首份心電圖完成份數(shù)、經(jīng)皮冠狀動(dòng)脈介入治療(PCI)支架置入情況、入門(mén)球囊擴(kuò)張時(shí)間(D2B)、住院天數(shù)及院內(nèi)死亡率等。結(jié)果與胸痛中心成立前比較,胸痛中心成立后患者的疾病種類構(gòu)成以及年齡、性別等差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),與對(duì)照組相比,觀察組第一份心電圖平均完成時(shí)間縮短(P=0.001)。觀察組PCI支架置入成功率高于對(duì)照組,但差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.222),入門(mén)球囊擴(kuò)張時(shí)間(P0.001)及住院時(shí)間(P=0.005)短于對(duì)照組,兩組院內(nèi)死亡率差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論胸痛中心的建立有效縮短了STEMI患者的救治時(shí)間,提高了治療效率,縮短了住院時(shí)間,值得在臨床中進(jìn)一步推廣。
[Abstract]:Objective to investigate the effect of the establishment of chest pain center on the treatment of (STEMI) patients with acute ST segment elevation myocardial infarction. Methods according to the requirements of the International chest pain Center Association, the chest pain center and the corresponding management system and treatment process were established. 1088 patients with acute ST segment elevation myocardial infarction (STEMI) treated in Shenyang military region General Hospital from May 2015 to March 2016 were selected as observation group and STEMI patients admitted from January to December 2014 as control group. There were 576 cases in the observation group and 512 cases in the control group. A retrospective cohort study was conducted. The observation group entered the chest pain center after admission, and the control group received routine treatment before the establishment of the chest pain center. The general condition of the two groups of patients with STEMI, the time of the first electrocardiogram (ECG) after admission, the number of the first electrocardiogram (ECG) completed in 10min, the implantation of (PCI) stents in percutaneous coronary intervention (PCI), and the time of initial balloon dilation (D2B) were observed. Hospital days and hospital mortality. Results there was no significant difference in the types of diseases, age, sex and so on between the patients after the establishment of the chest pain center (P0.05), compared with the control group. The average completion time of the first electrocardiogram in the observation group was shortened (P0. 001). The success rate of PCI stent implantation in the observation group was higher than that in the control group, but the difference was not statistically significant (P0. 222). The time of initial balloon dilation (P0. 001) and hospital stay (P0. 005) was shorter than that in the control group. There was no significant difference in hospital mortality between the two groups (P0.05). Conclusion the establishment of chest pain center can effectively shorten the treatment time of STEMI patients, improve the treatment efficiency and shorten the hospitalization time.
