【摘要】：目的觀(guān)察紅外熱療對(duì)于預(yù)防深二度燒傷創(chuàng)面銅綠假單胞菌感染的臨床療效。方法選取2015年1月至2016年10月筆者單位燒傷科收治的43例以深二度創(chuàng)面為主的燒傷住院患者,隨機(jī)將患者分配至磺胺嘧啶銀聯(lián)合創(chuàng)面紅外熱療組(簡(jiǎn)稱(chēng)試驗(yàn)組)及磺胺嘧啶銀組(簡(jiǎn)稱(chēng)對(duì)照組)。在治療前和治療后第3、7、14、21、28天,行創(chuàng)面分泌物評(píng)分、銅綠假單胞菌培養(yǎng)、創(chuàng)面愈合率、創(chuàng)面愈合時(shí)間及創(chuàng)面總體評(píng)價(jià)等指標(biāo),同時(shí)觀(guān)察不良反應(yīng),最后對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果 (1)兩組患者在年齡、性別、燒傷總面積等方面比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P0.05)。(2)治療第3、7、14天試驗(yàn)創(chuàng)面分泌物評(píng)分分別為(2.3±0.6、1.7±0.5、1.0±0.6),明顯低于常規(guī)治療組(2.8±0.4、2.1±0.7、1.7±0.7)(均P0.05);治療前及治療第21、28天,兩組患者創(chuàng)面分泌物評(píng)分相近。(3)兩組綠膿桿菌檢出率在治療第7、14、21天試驗(yàn)組銅綠假單胞菌陽(yáng)性率分別為(8/22、6/22、0/22),明顯低于對(duì)照組(18/21、15/21、4/21)(均P0.05)。(4)治療后第7、14、21天,試驗(yàn)組創(chuàng)面愈合率分別為[(39.2±11.1)%、(71.3±22.0)%、(94.6±7.3)%],高于對(duì)照組[(22.9±13.1)%、(54.8±19.9)%、(87.6±9.8)%],各組間比較差異有統(tǒng)計(jì)學(xué)意義(均P0.05);試驗(yàn)組患者創(chuàng)面愈合時(shí)間[(21.1±6.5)d]明顯短于對(duì)照組[(26.2±6.5)d,P0.05]。(5)治療后第14和21天試驗(yàn)組治療總體評(píng)價(jià)優(yōu)于對(duì)照組(均P0.05)。結(jié)論創(chuàng)面紅外熱療通過(guò)減少深二度燒傷創(chuàng)面分泌物,從而有效控制銅綠假單胞菌定植感染,進(jìn)而有助于創(chuàng)面愈合,達(dá)到一定縮短愈合時(shí)間的作用。
[Abstract]:Objective to observe the clinical effect of infrared hyperthermia on prevention of Pseudomonas aeruginosa infection in deep second degree burn wounds. Methods from January 2015 to October 2016, 43 patients with deep second degree burn were selected. Patients were randomly assigned to silver sulfadiazine combined with infrared hyperthermia group (test group) and silver sulfadiazine group (control group). The score of wound secretion, the culture of Pseudomonas aeruginosa, the rate of wound healing, the time of wound healing and the overall evaluation of wound were observed before and 28 days after treatment. At the same time, the adverse reactions were observed and analyzed statistically. Results (1) there was no significant difference in age, sex and total burn area between the two groups (P0.05). On the 14th day, the score of wound secretion was (2.3 鹵0.6) 鹵0.5 鹵1.0 鹵0.6, which was significantly lower than that in the routine treatment group (2.8 鹵0.4 鹵2.1 鹵0.71.7 鹵0.7) (P0.05). Before treatment and on the 21st day of treatment, the scores of wound secretion in the two groups were similar. (3) the positive rate of Pseudomonas aeruginosa in the test group was 8 / 22 / 226 / 22 / 22 on day 7, 14 and 21, respectively. Compared with the control group (18 / 21, 15 / 21, 4 / 21) (P0.05). (4), the wound healing rate in the experimental group was (39.2 鹵11.1)%, (71.3 鹵22.0)%, (94.6 鹵7.3)%, respectively. It was higher than the control group [(22.9 鹵13.1)%, (54.8 鹵19.9)%, (87.6 鹵9.8)%]. The wound healing time in the test group [(21.1 鹵6.5) days] was significantly shorter than that in the control group [(26.2 鹵6.5) days]. (5) the overall evaluation of treatment in the experimental group was better than that in the control group on the 14th and 21st days after treatment (P0.05). Conclusion Infrared hyperthermia can effectively control the infection of Pseudomonas aeruginosa by reducing the secretion of deep second degree burn wound, which can help the wound healing and shorten the healing time.
