彌散張量成像評估急性腦梗死康復治療前后運動功能變化與預后的相關性

【摘要】:目的利用磁共振彌散張量成像(DTI)和神經(jīng)功能評定量表(FMA)研究急性腦梗死患者康復治療前后CST類型、部分各向異性(FA)值變化與運動功能恢復及臨床預后之間的關系,探討影響腦梗死患者神經(jīng)功能恢復的相關因素。方法 40例急性腦梗死患者,隨機分為治療組與康復組。在發(fā)病3 d內(nèi)及發(fā)病3 m時行3.0T常規(guī)磁共振(MRI)和DTI檢查,分別測量病灶側(cè)、對側(cè)皮質(zhì)脊髓束FA值。通過Fugl-Meyer量表(FMA)評分,探討影響預后的相關因素。結(jié)果 3 m后兩組患者FMA值評分均較入院時明顯增高(P0.05),康復組CSTⅡ型、CSTⅢ型患者的FMA值較治療同組有顯著提高(P0.05)。預后以CSTⅠ型最好,CSTⅡ型次之,CSTⅢ型最差。所有患者入院時病灶側(cè)FA值均較健側(cè)減低,FA值CSTⅠ型CSTⅡ型于CSTⅡ型,P0.05。3 m復查病灶側(cè)FA值仍低于健側(cè),且較入院時各型FA值均有下降。結(jié)論早期康復訓練有助于促進患者運動功能恢復。皮質(zhì)脊髓束受損重、白質(zhì)纖維斷裂者,運動功能恢復較差。但通過康復訓練結(jié)果仍好于未康復訓練者。FA值與神經(jīng)功能損害呈正相關。
[Abstract]:Objective to study the relationship between CST types, partial anisotropic (FA) changes and motor function recovery and clinical prognosis in patients with acute cerebral infarction before and after rehabilitation by using (DTI) and (FMA). Objective: to investigate the related factors affecting the recovery of neural function in patients with cerebral infarction. Methods 40 patients with acute cerebral infarction were randomly divided into treatment group and rehabilitation group. 3.0T conventional magnetic resonance (MRI) and DTI were performed within 3 days of onset and 3 m after onset. The FA values of the corticospinal tract of the lesion and the contralateral corticospinal tract were measured respectively. The factors related to prognosis were investigated by (FMA) score of Fugl-Meyer scale.
