【摘要】：目的探討血清N末端腦鈉肽前體(NT-pro BNP)和降鈣素原(PCT)水平在膿毒癥患者中的表達(dá)水平及相關(guān)性。方法采集120例膿毒癥患者和40例健康志愿者(對(duì)照組)。膿毒癥患者按病情嚴(yán)重程度分為一般膿毒癥組、嚴(yán)重膿毒癥組、膿毒癥休克組,每組各40例。按患者入院28d后預(yù)后情況,又分為存活組和死亡組。記錄血清NT-pro BNP、PCT水平及急性生理和慢性健康狀況(APACHE-Ⅱ)評(píng)分。結(jié)果與對(duì)照組相比,膿毒癥各組的血清NT-pro BNP、PCT水平均明顯增高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);隨著膿毒癥病情的加重,血清NT-pro BNP、PCT水平和APACHEⅡ評(píng)分明顯升高,一般膿毒癥組、嚴(yán)重膿毒癥組、膿毒癥休克組兩兩比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。死亡組患者的血清PCT、NT-pro BNP水平和APACHEⅡ評(píng)分明顯高于存活組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。血清NT-pro-BNP靈敏度和特異度為81.34%、86.27%,PCT為83.08%、88.61%。血清NT-proBNP水平和血清PCT水平呈正相關(guān)(r=0.826,P0.05),血清NT-proBNP水平與APACHEⅡ評(píng)分呈正相關(guān)(r=0.756,P0.05),血清PCT水平與APACHEⅡ評(píng)分也呈正相關(guān)(r=0.702,P0.05)。結(jié)論血清NT-pro BNP和PCT水平對(duì)膿毒血癥患者病情程度和生存情況的預(yù)測(cè)評(píng)估具有重要的臨床應(yīng)用價(jià)值。
