【摘要】：目的探討米力農(nóng)微量泵入治療急性心肌梗死后心力衰竭患者的效果及其對(duì)血流動(dòng)力學(xué)的影響。方法選取62例急性心肌梗死后心力衰竭患者,隨機(jī)分成觀察組和對(duì)照組。在給予常規(guī)治療的基礎(chǔ)上,對(duì)照組采取西地蘭治療,西地蘭注射液0.025 mg/(kg·d)加入到0.9%氯化鈉注射液20 m L中靜注,1次/d,7 d為1個(gè)療程。觀察組采用米力農(nóng)治療,0.375~0.75 mg/(kg·d)加入到0.9%的氯化鈉溶液50 m L中,微量泵泵入,1次/d,7 d為1個(gè)療程,對(duì)比兩組7 d后的治療療效、血流動(dòng)力學(xué)指標(biāo)、B型尿鈉肽及不良反應(yīng)。結(jié)果觀察組總有效率顯著高于對(duì)照組(P0.01),治療后的左室舒張末內(nèi)徑、左室射血分?jǐn)?shù)、心臟指數(shù)、體循環(huán)血管阻力等血流動(dòng)力學(xué)指標(biāo)優(yōu)于對(duì)照組(P均0.01),不良反應(yīng)總發(fā)生率高于對(duì)照組(P0.01)。各組B型尿鈉肽、舒張壓、收縮壓、心率較治療前有顯著改善,以觀察組為著(P均0.05或0.01)。結(jié)論米力農(nóng)微量泵入能明顯改善急性心肌梗死后心力衰竭患者的臨床療效及血流動(dòng)力學(xué),但不良反應(yīng)稍高。
[Abstract]:Objective to investigate the effect of milrinone micropump in the treatment of heart failure after acute myocardial infarction and its effect on hemodynamics. Methods Sixty-two patients with heart failure after acute myocardial infarction were randomly divided into observation group and control group. On the basis of routine treatment, the control group was treated with cilidian. The injection of cilidian 0.025 mg/ (kg d) was injected intravenously into 0.9% sodium chloride injection 20 mL, once a day for 7 days as a course of treatment. The observation group was treated with milrinone, 0.375 and 0.75 mg/ (kg d) added to 0.9% sodium chloride solution 50 mL, the micropump was pumped in, once a day for 7 days as a course of treatment. The therapeutic effect, hemodynamic index, type B urinary natriuretic peptide and adverse reactions were compared between the two groups after 7 days. Results the total effective rate of the observation group was significantly higher than that of the control group (P0.01). The left ventricular end-diastolic diameter, left ventricular ejection fraction, cardiac index after treatment were significantly higher in the observation group than in the control group (P0.01). The hemodynamic indexes such as systemic vascular resistance were better than those of the control group (P0.01), and the total incidence of adverse reactions was higher than that of the control group (P0.01). Type B natriuretic peptide, diastolic blood pressure, systolic blood pressure and heart rate were significantly improved in the observation group (P 0.05 or 0.01). Conclusion milrinone micropump can significantly improve the clinical efficacy and hemodynamics of patients with heart failure after acute myocardial infarction, but the adverse reaction is slightly higher.
