微小RNA對中國急性心肌梗死患者早期診斷價值的Meta分析

【摘要】:目的:評價微小RNA(miRNAs)對中國急性心肌梗死(AMI)患者的早期診斷價值。方法:采用Cochrane系統(tǒng)評價的基本方法,全面檢索Pubmed、Medline、Web of science、中國期刊全網(wǎng)全文數(shù)據(jù)庫(CNKI)、萬方數(shù)據(jù)庫、中國生物醫(yī)學文獻數(shù)據(jù)庫,無語言限制,搜集評價miRNAs對中國AMI患者診斷價值的文獻,以靈敏度(Sen)、特異度(Spe)、診斷優(yōu)勢比(DOR)及受試者工作特征曲線下面積(SROC)作為診斷效能的評價指標,采用MetaDisc1.4軟件進行系統(tǒng)評價和Meta分析。結果:共納入24篇文獻,其中英文15篇,中文9篇。Meta分析結果顯示:總miRNAs的合并Sen、Spe和SROC面積分別為0.73(95%CI:0.72~0.75)、0.82(95%CI:0.81~0.84)和0.8783;miRNA-499的合并Sen、Spe和SROC面積為0.78(95%CI:0.75~0.81)、0.76(95%CI:0.72~0.79)和0.8781;miRNA-1的合并Sen、Spe和SROC面積為0.73(95%CI:0.69~0.78)、0.84(95%CI:0.80~0.87)和0.8374;miRNA-208的合并Sen、Spe和SROC面積為0.73(95%CI:0.68~0.77)、0.85(95%CI:0.80~0.88)和0.8658;miRNA-133的合并Sen、Spe和SROC面積為0.75(95%CI:0.70~0.80)、0.88(95%CI:0.84~0.92)和0.9391。結論:miRNA是一種新型的心臟損傷標志物,尤其miRNA-133對早期AMI具有較強的診斷價值,但仍需要更大樣本高質量的臨床試驗加以驗證。
