【摘要】：目的:了解中國(guó)急性ST段抬高型心肌梗死(STEMI)行經(jīng)皮冠狀動(dòng)脈介入治療(PCI)患者負(fù)荷劑量氯吡格雷的應(yīng)用情況,并探討其影響因素。方法:基于"冠心病醫(yī)療結(jié)果評(píng)價(jià)和臨床轉(zhuǎn)化研究—回顧性急性心肌梗死研究",收集2006年和2011年因STEMI住院并接受PCI治療患者的臨床信息,計(jì)算不同年份患者水平和醫(yī)院水平負(fù)荷劑量氯吡格雷(將單次使用劑量≥300 mg且450 mg者納入300 mg組,將≥450 mg且≤600 mg者納入600 mg組)的應(yīng)用情況,并采用二元Logistic回歸方法分析其使用的影響因素。結(jié)果:共入選2 481例患者,平均年齡(60.9±12.0)歲,女性占21.4%。2006年至2011年,使用負(fù)荷劑量氯吡格雷總體(51.0%比47.4%)、300 mg負(fù)荷劑量(43.1%比39.2%)和600 mg負(fù)荷劑量(7.8%比8.2%)的患者比例均無(wú)明顯改變(P均0.05)。2006年,醫(yī)院水平的中位使用率為44.4%[四分位間距(IQR):21.8%至69.0%],2011年,中位使用率為48.1%(IQR:25.0%至70.8%),兩年份的中位使用率差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.940)。多因素分析顯示,發(fā)病至入院時(shí)間小于12 h、行直接PCI和在中部地區(qū)就診的患者負(fù)荷劑量使用率較高。結(jié)論:中國(guó)接受PCI治療的STEMI患者中負(fù)荷劑量氯吡格雷應(yīng)用嚴(yán)重不足,醫(yī)院間差異明顯且2011年較2006年未見(jiàn)改善。負(fù)荷劑量氯吡格雷在此類患者中的應(yīng)用亟待改善。
