【摘要】：目的探討早期腦鈉肽(BNP)、C反應(yīng)蛋白(CRP)、乳酸(Las)水平聯(lián)合檢查評(píng)估老年感染性休克患者預(yù)后的臨床價(jià)值。方法回顧性分析2012年6月至2015年3月該院重癥監(jiān)護(hù)室(ICU)治療的75例老年感染性休克患者的臨床資料,依據(jù)28 d后的存活情況分為存活組(41例)和死亡組(34例),分析不同組別BNP、CRP、Las的差異,并采用受試者工作曲線(ROC)進(jìn)一步分析。結(jié)果死亡組BNP、CRP、Las水平均高于生存組(P0.05);約登指數(shù)(Youden)最大原則確定BNP、CRP、Las評(píng)估最佳臨界值為:13 310.47 pg/ml、23 mg/L、3.1 mmol/L,此時(shí)評(píng)估"死亡"的靈敏度、特異度、準(zhǔn)確率分別為:82.35%、78.05%、80.00%;67.64%、70.73%、69.33%;79.41%、87.80%、84.00%。三種指標(biāo)聯(lián)合評(píng)估時(shí),≥1種、≥2種、3種高于臨界值的靈敏度、特異度、準(zhǔn)確率分別為:97.06%、43.90%、68.00%;88.24%、85.37%、86.67%;58.82%、95.12%、78.67%。結(jié)論早期聯(lián)合檢測(cè)BNP、CRP、Las有助于評(píng)估老年感染性休克患者預(yù)后,這3種中任意≥2種高于臨界值時(shí)多提示預(yù)后不佳。
[Abstract]:Objective to evaluate the clinical value of early brain natriuretic peptide (BNP) (BNP) C-reactive protein (CRP),) lactate (Las) level in evaluating the prognosis of elderly patients with septic shock. Methods the clinical data of 75 elderly patients with septic shock treated by (ICU) from June 2012 to March 2015 were retrospectively analyzed.
