【摘要】：目的探討ERK信號通路對急性ST段抬高型心肌梗死患者急診PCI術(shù)后冠脈無復(fù)流的影響。方法選擇急性ST段抬高型心肌梗死(STEMI)行急診PCI的患者38例,根據(jù)術(shù)后TIMI分級分為無復(fù)流患者16例(無復(fù)流組)和有效灌注者22例(再灌注組);另選擇20例健康體檢者作為對照組。采用全自動生化分析儀測定各組血清總膽固醇(TC)甘油三酯(TG)低密度脂蛋白膽固醇(LDL-C)高密度脂蛋白膽固醇尿素氮肌酐尿酸等生化指標(biāo),利用RT-PCR方法測定各組外周血單個核細(xì)胞ERK mRNA及bad mRNA表達(dá)。結(jié)果無復(fù)流組、再灌注組血清TG、TC、LDL-C水平明顯高于對照組(P均0.05);無復(fù)流組血清LDL-C水平明顯高于再灌注組(P0.05)。對照組單個核細(xì)胞ERK mRNA表達(dá)水平高于無復(fù)流組及再灌注組,bad mRNA表達(dá)水平低于無復(fù)流組及再灌注組(P均0.05);無復(fù)流組ERK mRNA表達(dá)水平低于再灌注組,bad mRNA表達(dá)水平高于再灌注組(P均0.05)。單個核細(xì)胞ERK mRNA與bad mRNA表達(dá)水平呈負(fù)相關(guān)(r=-0.61,P0.05)。結(jié)果在STEMI患者急診PCI術(shù)無復(fù)流的患者中ERK信號通路表達(dá)活性下降;ERK信號通路活性降低及bad mRNA表達(dá)增強(qiáng)可能參與了急診PCI術(shù)后冠脈無復(fù)流的發(fā)生。
[Abstract]:Objective to investigate the effect of ERK signal pathway on coronary artery free reflow after emergency PCI operation in patients with acute ST elevation myocardial infarction. Methods 38 patients with acute ST segment elevation myocardial infarction (STEMI) were selected for emergency PCI. According to the postoperative TIMI classification, 16 patients without reflow (no reflow group) and 22 patients with effective perfusion (reperfusion group) were selected, and 20 were selected. The serum total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL), urea nitrogen, creatinine and uric acid were measured by a full automatic biochemical analyzer, and the expression of ERK mRNA and bad mRNA in peripheral blood mononuclear cells in each group was measured by RT-PCR method. The serum levels of TG, TC and LDL-C in the reperfusion group were significantly higher than those in the control group (P 0.05), and the serum LDL-C level in the non reflow group was significantly higher than that in the reperfusion group (P0.05). The expression level of ERK mRNA in the mononuclear cells in the control group was higher than that without reflow group and reperfusion group. The level of bad mRNA in the control group was lower than that in the non reflow group and the reperfusion group (P 0.05); no reflow was found. The expression level of ERK mRNA was lower than that in the reperfusion group, and the expression level of bad mRNA was higher than that in the reperfusion group (P 0.05). The ERK mRNA in mononuclear cells was negatively correlated with the mRNA expression level of bad (r=-0.61, P0.05). Strong may be involved in the occurrence of coronary no reflow after emergency PCI.
