【摘要】：目的對5年筆者單位燒傷患者細菌分布及耐藥性變化進行回顧性分析,為今后防治感染提供參考。 方法收集2007年1月-2011年12月筆者單位燒傷病房537例燒傷患者各類標本中分離的細菌,對細菌分布及耐藥性情況進行分析。 結(jié)果革蘭陰性桿菌共637株、占62.76%,檢出率呈上升趨勢；革蘭陽性球菌共323株、占31.82%,檢出率呈下降趨勢。檢出率由高到低前8位的細菌依次是：銅綠假單胞菌、金黃色葡萄球菌、鮑氏不動桿菌、肺炎克雷伯桿菌、大腸埃希菌、陰溝腸桿菌、凝固酶陰性葡萄球菌、奇異變形桿菌。銅綠假單胞菌對頭孢他啶、頭孢哌酮/舒巴坦、亞胺培南耐藥率分別為34.27%、40.88%、26.29%。鮑氏不動桿菌除對頭孢哌酮/舒巴坦較敏感外(耐藥率為29.51%),對其他抗生素廣泛耐藥。多重耐藥/泛耐藥銅綠假單胞菌(multidrug-resistant/pandrug-resistant Pesudomonas aemginosa, MDR/PDR-PA)及耐碳青霉烯類/泛耐藥鮑氏不動桿菌(Carbapenems-resistant/pandrug-resistant Acinetobacter baumannii, CR/PDR-AB)檢出率呈明顯上升趨勢。金黃色葡萄球菌以多重耐藥的MRSA為主,其對萬古霉素、利奈唑胺、替考拉寧的耐藥率分別為為0、0、5.71%。MRSA檢出率呈下降趨勢。 結(jié)論筆者單位主要細菌多重耐藥/泛耐藥現(xiàn)象日益嚴重,臨床應(yīng)予以重視并合理用藥。
[Abstract]:Objective to analyze the changes of bacterial distribution and drug resistance in burn patients for 5 years and to provide reference for prevention and treatment of infection in the future. Methods the bacteria isolated from 537 burn patients from January 2007 to December 2011 were collected and the distribution and drug resistance of bacteria were analyzed. Results there were 637 gram-negative bacilli (62.76), and 323 gram-positive cocci (31.82%) showed a decreasing trend. Pseudomonas aeruginosa, Staphylococcus aureus, Acinetobacter baumannii, Klebsiella pneumoniae, Escherichia coli, Enterobacter cloacae, coagulase negative Staphylococcus, Proteus mirabilis were the first 8 bacteria in the detection rate. The resistance rates of Pseudomonas aeruginosa to ceftazidime, cefoperazone / sulbactam and imipenem were 34.27%, 40.88% and 26.29%, respectively. Acinetobacter baumannii was sensitive to cefoperazone / sulbactam (29.51%) and widely resistant to other antibiotics. The detection rates of multidrug-resistant/pandrug-resistant Pesudomonas aemginosa (MDR / PDR-PA) and Carbapenems-resistant / pan-resistant Acinetobacter baumannii (CRPDR-AB) increased significantly. MRSA was mainly multidrug resistant staphylococcus aureus. The drug resistance rates of MRSA to vancomycin, linazolamine and teicoplanin were 0 0, 5. 71. MRSA respectively. The positive rate of MRSA decreased. Conclusion the phenomenon of multidrug resistance / pan-resistance of the main bacteria is becoming more and more serious, which should be paid more attention to and used rationally in clinic.
