【摘要】：目的探討超聲測(cè)量在評(píng)價(jià)體外沖擊波治療跖腱膜炎效果中的應(yīng)用。方法應(yīng)用超聲觀察和測(cè)量臨床診斷為跖腱膜炎的21例中老年患者(患者組)的患側(cè)跖腱膜跟骨附著部的厚度,并測(cè)量35例正常中老年人的單側(cè)跖腱膜相應(yīng)部位作為對(duì)照(正常組)。其中,患者組于體外沖擊波首次治療前和治療1個(gè)療程(每周1次治療,5次/療程)后各測(cè)量1次,并進(jìn)行VAS評(píng)分以評(píng)估療效。結(jié)果治療前,患者組患足的跖腱膜跟骨附著部的厚度為(0.582±0.013)cm,較正常組的厚度(0.333±0.001)cm明顯增加(P0.01)。經(jīng)1個(gè)療程沖擊波治療后,患者組腱膜跟骨附著部的厚度減少為(0.517±0.013)cm,與治療前相比差異有統(tǒng)計(jì)學(xué)意義(P0.01),但仍大于正常組的厚度(P0.01);且VAS評(píng)分較治療前明顯減少,治療有效率達(dá)57.28%(P0.01)。結(jié)論超聲測(cè)量用于評(píng)價(jià)體外沖擊波治療跖腱膜炎的效果確切,且兼具無(wú)創(chuàng)、便捷等優(yōu)點(diǎn)。
[Abstract]:Objective to evaluate the effect of extracorporeal shock wave on plantar aponeurosis. Methods Ultrasound was used to observe and measure the thickness of calcaneal attachment of the metatarsal aponeurosis in 21 middle and aged patients (patient group) diagnosed clinically as metatarsal aponeurosis. The corresponding position of unilateral metatarsal aponeurosis was measured in 35 normal aged people as control group (normal group). The patients were measured before the first treatment of extracorporeal shock wave (ESWL) and 1 course of treatment (5 times a week), and VAS scores were used to evaluate the efficacy. Results before treatment, the thickness of calcaneal attachment of metatarsal aponeurosis in the patients group was 0.582 鹵0.013 cm, which was significantly higher than that in the normal group (0.333 鹵0.001)cm). After one course of shock wave treatment, the thickness of aponeurosis calcaneal attachment in the patients group decreased to 0.517 鹵0.013 cm, which was significantly different from that before treatment, but still larger than that of the normal group, and the VAS score was significantly reduced compared with that before treatment, and the effective rate was 57.28% (P0.01). Conclusion Ultrasonography can be used to evaluate the effect of extracorporeal shock wave in the treatment of plantar aponeurosis.
