【摘要】：目的觀察急性心肌梗死患者及正常人血清生長分化因子-15(GDF-15)水平的變化及其與心肌梗死后左房前后徑、左室舒張末徑、左室質(zhì)量指數(shù)、左室相對(duì)室壁厚度、射血分?jǐn)?shù)的相關(guān)性,探討GDF-15與急性心肌梗死后左室重構(gòu)程度的相關(guān)性。 方法選取急性心肌梗死患者50例分為兩組,左心室重構(gòu)組30例,無左心室重構(gòu)組20例。正常對(duì)照組30例。采集外周靜脈血,采用酶聯(lián)免疫吸附法檢測(cè)血清中GDF-15水平。超聲心動(dòng)圖測(cè)定左房前后徑、左室舒張末徑、舒張末左室后壁厚度、舒張末室間隔厚度,射血分?jǐn)?shù)等左室重構(gòu)指標(biāo)。 結(jié)果1.左心室重構(gòu)組GDF-15的表達(dá)水平和無左心室重構(gòu)組的GDF-15濃度均高于正常對(duì)照組(P0.01)。左心室重構(gòu)組的GDF-15濃度高于無左心室重構(gòu)組(P0.01)。 2.GDF-15濃度與年齡、性別、體重、高血壓病、高血脂、糖尿病、吸煙、飲酒、高密度脂蛋白、低密度脂蛋白、甘油三酯、高敏C反應(yīng)蛋白均不相關(guān),與總膽固醇呈負(fù)相關(guān)(P=0.04),與腦鈉肽呈正相關(guān)(P0.01)。 3.GDF-15與左房前后徑(r=0.745,P0.01)、左室舒張末徑(r=0.84,P0.01)、左室質(zhì)量指數(shù)(r=0.993,P0.01)呈正相關(guān)；與射血分?jǐn)?shù)(r=-0.65,P0.01)、相對(duì)室壁厚度(r=-0.96,P0.01)呈負(fù)相關(guān)。 4.采取多元線性回歸分析,控制了年齡、性別和心血管危險(xiǎn)因素后,GDF-15水平與左室質(zhì)量指數(shù)呈正相關(guān)(B=0.91,P0.01)。 結(jié)論GDF-15在急性心肌梗死的心肌細(xì)胞中高度表達(dá),參與急性心肌梗死后左室重構(gòu)的過程,其水平與左室重構(gòu)程度呈顯著的、獨(dú)立的相關(guān)性。
[Abstract]:Objective to observe the changes of serum GDF-15 levels in patients with acute myocardial infarction (AMI) and normal subjects and their correlation with left atrial diameter, left ventricular end-diastolic diameter, left ventricular mass index, left ventricular relative wall thickness and ejection fraction. To investigate the correlation between GDF-15 and left ventricular remodeling after acute myocardial infarction. Methods Fifty patients with acute myocardial infarction were divided into two groups: the left ventricular remodeling group (30 cases) and the non-left ventricular remodeling group (20 cases). 30 cases in normal control group. Peripheral venous blood was collected and serum GDF-15 level was detected by enzyme linked immunosorbent assay (Elisa). Left ventricular remodeling indexes such as left atrial anteroposterior diameter left ventricular end-diastolic diameter left ventricular posterior wall thickness end-diastolic septal thickness ejection fraction and so on were measured by echocardiography. Result 1. The expression of GDF-15 in the left ventricular remodeling group and the GDF-15 concentration in the group without left ventricular remodeling were higher than those in the normal control group (P 0.01). The concentration of GDF-15 in the left ventricular remodeling group was higher than that in the no left ventricular remodeling group (P 0.01). 2.GDF-15 concentrations were not associated with age, sex, body weight, hypertension, hyperlipidemia, diabetes, smoking, alcohol consumption, HDL, LDL, triglyceride, Gao Min C-reactive protein. There was a negative correlation with total cholesterol and a positive correlation with brain natriuretic peptide (BNP). There was a positive correlation between 3.GDF-15 and left atrial anterior diameter (RV), left ventricular end-diastolic diameter (LVDD), left ventricular mass index (LVMI) and left ventricular mass index (LVMI), and a negative correlation was found between 3.GDF-15 and ejection fraction (r = -0.65U, P = 0.01a, relative wall thickness r = 0.96P 0.01). 4. Multivariate linear regression analysis was used to control age, sex and cardiovascular risk factors. GDF-15 levels were positively correlated with left ventricular mass index (LVMI). Conclusion the high expression of GDF-15 in myocardial cells after acute myocardial infarction is involved in the process of left ventricular remodeling, and its level is significantly correlated with the degree of left ventricular remodeling.
