【摘要】：目的觀察高壓氧（HBO）聯(lián)合經(jīng)顱磁刺激（TMS）對(duì)急性腦梗死后神經(jīng)功能及焦慮狀態(tài)的治療效果。 方法選取2011年1月-2012年12月在寧夏自治區(qū)人民醫(yī)院神經(jīng)內(nèi)科住院的腦梗死患者134例，所用病例診斷均符合全國(guó)第四屆腦血管疾病學(xué)術(shù)會(huì)議制定的診斷標(biāo)準(zhǔn)，且入院時(shí)漢密爾頓焦慮量表（HAS）評(píng)分14分。入院后，對(duì)所有病例的年齡、性別、文化程度、家庭人均收入、婚姻狀況、陪護(hù)情況進(jìn)行詳細(xì)記錄。將所收集病例分為對(duì)照組47例、HBO組44例、HBO+TMS組43例。對(duì)照組給予常規(guī)藥物治療，HBO組在常規(guī)藥物治療基礎(chǔ)上加HBO治療；HBO+TMS組給予常規(guī)藥物、HBO及TMS治療。采用歐洲卒中量表（ESS）、日常生活活動(dòng)能力量表（ADL）、漢密爾頓焦慮量表（HAS）分別于治療前和治療20d后進(jìn)行評(píng)定。結(jié)果⑴20d后，對(duì)照組、HBO組、HBO+TMS組焦慮的總有效率分別53.19%，77.27% 和90.70%，三組之間比較有顯著性差異（X~2=16.648，P0.05）。⑵對(duì)照組、HBO組、HBO+TMS組ESS評(píng)分分別為72.15±6.62，79.02±6.99及83.09 ±6.12；ADL評(píng)分分別為27.55±4.33，22.55±3.72及19.26±3.16；HAS評(píng)分分別為16.11±4.76，14.16±3.62及12.16±4.46，三組ESS、ADL、HAS評(píng)分與治療前比較有統(tǒng)計(jì)學(xué)意義（P0.05）；三組之間各項(xiàng)評(píng)分亦有統(tǒng)計(jì)學(xué)意義（P0.05）。⑶經(jīng)秩和檢驗(yàn)，婚姻狀況、年齡、家庭人均收入、文化程度與焦慮嚴(yán)重程度有關(guān) （P0.05），而性別、陪護(hù)與焦慮程度無(wú)關(guān)（P0.05）。結(jié)論高壓氧聯(lián)合經(jīng)顱磁刺激對(duì)急性腦梗死后焦慮狀態(tài)短期內(nèi)療效確切，不僅可以 改善患者焦慮狀態(tài)、促進(jìn)患者神經(jīng)功能恢復(fù)，而且還可以提高腦梗死患者的生活質(zhì)量，且未發(fā)生不良反應(yīng)。
[Abstract]:Objective to observe the effect of HBO combined with transcranial magnetic stimulation (TMS) on nerve function and anxiety after acute cerebral infarction. Methods 134 patients with cerebral infarction were selected from January 2011 to December 2012 in Department of Neurology, people's Hospital of Ningxia Autonomous region. At admission, Hamilton anxiety scale (HAS14) was scored. After admission, all cases of age, sex, education, family income per capita, marital status, escort status were recorded in detail. The patients were divided into control group (n = 47) and TMS group (n = 44). The control group was treated with routine drug therapy plus HBO on the basis of routine drug therapy. The TMS group was treated with routine drugs such as HBO and TMS. The European Stroke scale (ESS), activity of Daily living (ADL) and Hamilton anxiety scale (HASS) were used to evaluate before and 20 days after treatment. Results after 120 days, the total effective rate of anxiety in the control group was 53.19% and 77.27%, respectively. There were significant differences among the three groups. The ESS scores of TMS group were 72.15 鹵6.62 鹵6.62 鹵6.99 and 83.09, respectively. The scores of ADL in the three groups were 27.55 鹵4.33 鹵22.55 鹵3.72 and 19.26 鹵3.16 respectively, 16.11 鹵4.76 鹵14.16 鹵3.62 and 12.16 鹵4.46, respectively. The degree of education is related to the severity of anxiety. There was no correlation between the degree of anxiety and sex, but there was no relationship between the sex and the degree of anxiety. Conclusion Hyperbaric oxygen combined with transcranial magnetic stimulation is effective in the treatment of anxiety after acute cerebral infarction. It can also improve the quality of life of patients with cerebral infarction without adverse reactions.
