【摘要】：本文選取了2016年4月~2017年2月于海口市人民醫(yī)院急診和門診首診的急性心力衰竭組100例,和健康查體中心健康組100例,分別符合納入標(biāo)準(zhǔn)進(jìn)行分組比較。心力衰竭臨床診斷標(biāo)準(zhǔn)符合《2016年歐洲心臟病學(xué)會(huì)急性心力衰竭指南》。進(jìn)行ELISA法檢測兩組血清PTH、NT-pro BNP,分別檢測和聯(lián)合檢測兩組的血清PTH和NT-proBNP。同時(shí)超聲心動(dòng)監(jiān)測心臟血流動(dòng)力學(xué)指標(biāo),再以不同左心室射血分?jǐn)?shù)下分組,分別統(tǒng)計(jì)單獨(dú)檢測血清PTH和NT-proBNP和聯(lián)合檢測兩者的敏感度、特異度。結(jié)果顯示:單獨(dú)檢測血清PTH和NT-proBNP,急性心力衰竭組與健康組比較,統(tǒng)計(jì)有差異(P0.05)。分別比較兩組的聯(lián)合檢測血清PTH和NT-proBNP,急性心力衰竭組在左心室射血分?jǐn)?shù)LVEF≥50%時(shí)既表現(xiàn)出統(tǒng)計(jì)學(xué)差異(P0.05),且敏感度和特異度高。以上說明血清PTH和NT-proBNP聯(lián)合檢測提高心衰實(shí)驗(yàn)室檢查敏感性,并結(jié)合超聲血流動(dòng)力學(xué)指標(biāo)可提高早期未出現(xiàn)血流動(dòng)力學(xué)障礙時(shí)急性心力衰竭診斷準(zhǔn)確性。可有效判讀臨床結(jié)果并便于急診分級系統(tǒng)管理,尤其是社區(qū)門診監(jiān)測中射血分?jǐn)?shù)尚在代償范圍的心力衰竭患者,降低心力衰竭患者的漏診和誤診。從心功能和體內(nèi)電解質(zhì)代謝角度及早進(jìn)行多維度整體判斷心力衰竭患者病理生理變化,具有一定的臨床應(yīng)用價(jià)值。
[Abstract]:From April 2016 to February 2017, 100 patients with acute heart failure and 100 healthy people in the health examination center were selected from the emergency department and outpatient clinic of Haikou people's Hospital. The patients were divided into two groups according to the inclusion criteria. The clinical diagnostic criteria for heart failure are in accordance with the European Heart Association guidelines for Acute Heart failure 2016. The serum PTH and NT-proBNPs of the two groups were detected by ELISA assay. At the same time, the cardiac hemodynamic parameters were monitored by echocardiography. The sensitivity and specificity of serum PTH and NT-proBNP were measured separately and combined with different left ventricular ejection fractions (LVEF). The results showed that serum PTH and NT-proBNPs were detected separately, and there was a statistical difference between the acute heart failure group and the healthy group (P 0.05).
