動脈二氧化碳分壓監(jiān)測聯(lián)合被動抬腿試驗在評估循環(huán)衰竭患者容量反應(yīng)性的應(yīng)用研究

【摘要】:目的:探討動脈血二氧化碳分壓監(jiān)測聯(lián)合被動抬腿試驗在評估循環(huán)衰竭患者容量反應(yīng)性中的應(yīng)用價值,尋找更安全、可靠的評估容量反應(yīng)性的方法。 方法:納入32例循環(huán)衰竭患者,行脈搏指示持續(xù)心輸出量(PiCCO)監(jiān)測血流動力學,每例患者行4分鐘的被動抬腿試驗(Passive Leg-raising PLR),分別記錄PLR前、PLR1分鐘時的心臟指數(shù)(CI),并同步測定PLR前、PLR1分鐘時的動脈血二氧化碳分壓(PaCO2)。PLR后在1小時內(nèi)給患者輸注平衡液500ml,記錄輸液前后的CI值。計算PLR前后PaCO2的變化值(ΔPaCO2-PLR)和CI的變化值(ΔCI-PLR)以及液體負荷前后的CI變化值(ΔCI-Fluid)。使用單變量一般線性模型對ΔPaCO2-PLR和ΔCI-PLR進行相關(guān)性分析評價二者的相關(guān)性,以明確PLR過程中PaCO2變化是否能反映心輸出量的變化;而后以ΔCI-Fluid≥15%為金標準,將病人分為液體負荷試驗陽性組和液體負荷試驗陰性組;以ΔPaCO2-PLR≥5%基礎(chǔ)值為陽性標準,又可將病人分為PLR-CO2陽性組和PLR-CO2陰性組,以液體負荷試驗組的陽性、陰性結(jié)果為金標準,與PLR-CO2組的陽性、陰性結(jié)果進行比較,列出四格表,算出以被動抬腿試驗后PaCO2升高≥5%為標準預(yù)測患者在隨后的液體負荷試驗中CI增加≥15%的敏感性及特異性,并通過SPSS軟件處理,得出以被動抬腿試驗前后PaCO2的變化來預(yù)測患者容量反應(yīng)性的ROC曲線,計算出其曲線下面積,評價其診斷價值,ROC曲線的曲線下面積表示為面積±標準差(95%置信區(qū)間)。 結(jié)果:32例患者完成試驗,PLR前后PaCO2的變化與心輸出量的變化有很強的正相關(guān)性(r=0.64, P<0.05),以ΔPaCO2≥5%為陽性標準,根據(jù)PLR前后PaCO2變化值預(yù)測患者在隨后的液體負荷試驗中CI升高≥15%的敏感性為82.1%,特異性100%,陽性預(yù)測值100%,陰性預(yù)測值44.4%,AUC為0.90(95%CI0.79-1.0,P<0.05),具有較高的診斷價值。 結(jié)論:動脈血二氧化碳分壓監(jiān)測聯(lián)合被動抬腿試驗對評估循環(huán)衰竭患者的容量反應(yīng)性有較大的診斷價值。
[Abstract]:Objective: to explore the application value of arterial blood carbon dioxide partial pressure monitoring combined with passive leg lifting test in evaluating volumetric reactivity in patients with circulatory failure, and to find a more safe and reliable method for evaluating volume reactivity. Methods: the hemodynamics of 32 patients with circulatory failure were monitored by pulse indicator continuous cardiac output (Pi CCO). Each patient underwent a four-minute passive leg lifting test and passive Leg-raising PLR. The cardiac index was recorded at 1 minute before PLR, and the arterial carbon dioxide partial pressure (Paco _ 2 路PLR) at 1 minute before PLR was measured simultaneously. The balanced fluid was infused within 1 hour. 500 ml, the CI value before and after infusion was recorded. The changes of PaCO2 (螖 Paco _ 2-PLR) and CI (螖 CI-PLR) before and after PLR and CI changes before and after liquid loading (螖 CI-Fluidn) were calculated.
