【摘要】：目的:探討重組人生長激素(HGH)對老年重度燒傷患者血清內(nèi)毒素、炎癥因子、創(chuàng)面愈合時間、住院時間及預(yù)后的影響。方法:選取在我院治療的老年重度燒傷患者96例,按隨機數(shù)字表法分為治療組和對照組,各48例。兩組患者均給予創(chuàng)傷面護理、營養(yǎng)支持,維持水、電解質(zhì)及酸堿平衡;對照組患者皮下注射0.9%氯化鈉注射液1 ml,qd;治療組患者給予注射用HGH 0.3 IU/kg皮下注射,qd。兩組患者均治療21 d。比較兩組患者治療前及治療14、21 d后的血清內(nèi)毒素含量,觀察兩組患者治療前后血清炎癥因子[腫瘤壞死因子(TNF)α、白細胞介素(IL)6、IL-8]水平和創(chuàng)面愈合時間、住院時間,計算兩組患者多器官障礙綜合征(MODS)、死亡、全身感染的發(fā)生率。結(jié)果:兩組患者治療前及治療14 d后的內(nèi)毒素含量比較,差異無統(tǒng)計學(xué)意義(P0.05);治療21 d后,兩組患者血清內(nèi)毒素含量明顯降低,且治療組患者明顯低于對照組,差異有統(tǒng)計學(xué)意義(P0.05)。治療后,治療組患者IL-8水平明顯高于對照組,TNF-α、IL-6明顯低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。治療組患者各創(chuàng)面愈合時間及住院時間均顯著短于對照組,差異均有統(tǒng)計學(xué)意義(P0.05);治療組患者MODS、死亡及全身感染的發(fā)生率均顯著低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:HGH能使老年重度燒傷患者吸收的毒素減少,改善炎癥因子水平,從而促進患者創(chuàng)面愈合及改善預(yù)后。
[Abstract]:Objective: to investigate the effects of recombinant human growth hormone (HGH) on serum endotoxin, inflammatory factors, wound healing time, hospital stay and prognosis in elderly patients with severe burn. Methods: 96 elderly patients with severe burn were randomly divided into treatment group (n = 48) and control group (n = 48). The patients in both groups were given trauma care, nutritional support, water, electrolyte and acid-base balance, the control group was subcutaneously injected with 0.9% sodium chloride injection 1 ml QD, and the treatment group was subcutaneously injected with HGH 0.3 IU/kg. The patients in both groups were treated for 21 days. The serum endotoxin levels before and after 14 days treatment were compared between the two groups. The levels of serum inflammatory factor (TNFa) 偽, interleukin-6 IL-8 (IL-8), wound healing time and hospitalization time were observed before and after treatment. The incidence of MODS, death and systemic infection in patients with multiple organ dysfunction syndrome were calculated. Results: there was no significant difference in endotoxin content between the two groups before treatment and 14 days after treatment, but after 21 days of treatment, the serum endotoxin level in the two groups was significantly lower than that in the control group, and the level of endotoxin in the treatment group was significantly lower than that in the control group. The difference was statistically significant (P 0.05). After treatment, the level of IL-8 in the treatment group was significantly higher than that in the control group, and the level of TNF- 偽 and IL-6 in the treatment group was significantly lower than that in the control group (P 0.05). The healing time and hospitalization time of each wound in the treatment group were significantly shorter than those in the control group (P 0.05), and the incidence of MODS, death and systemic infection in the treatment group were significantly lower than those in the control group (P 0.05). ConclusionHGH can reduce the absorption of toxin and improve the level of inflammatory factors in the elderly patients with severe burn, thus promoting wound healing and improving prognosis.
