【摘要】：目的分析重癥監(jiān)護病房(ICU)假絲酵母菌菌血癥患者的臨床和微生物特征,評價其死亡的預后危險因素,以指導臨床治療。方法回顧性分析2005年6月-2013年5月醫(yī)院ICU 69例假絲酵母菌菌血癥患者的臨床資料,分析病原菌種類、基礎(chǔ)疾病、危險因素、預后情況,使用單因素分析及多因素logistic回歸分析假絲酵母菌菌血癥死亡的相關(guān)危險因素;使用SPSS13.0軟件對數(shù)據(jù)進行統(tǒng)計分析。結(jié)果血培養(yǎng)共分離到69株假絲酵母菌屬,其中白色假絲酵母菌35株占50.7%,光滑假絲酵母菌12株占17.4%,近平滑假絲酵母菌6株占8.7%,熱帶假絲酵母菌5株占7.2%,經(jīng)單因素分析及多因素logistic回歸分析發(fā)現(xiàn),并發(fā)感染性休克(OR=8.601,95%CI為1.756~42.120)、合并細菌菌血癥(OR=32.393,95%CI為3.686~284.657)、低蛋白血癥(OR=25.496,95%CI為2.876~226.067)為死亡的獨立危險因素。結(jié)論 ICU假絲酵母菌菌血癥以白色假絲酵母菌為主,病死率高,應(yīng)針對危險因素采取相應(yīng)治療措施。
[Abstract]:Objective to analyze the ICU (ICU) clinical and microbiological characteristics of Candida bacteremia patients, to evaluate the prognostic factors of death, to guide clinical treatment. Methods Retrospective analysis of June 2005 May -2013 hospital ICU 69 cases of Candida bacteremia in patients with clinical data, analysis the types of pathogens, underlying diseases, risk factors the prognosis of logistic was analyzed using univariate and multivariate regression analysis of risk factors of death of yeast Candida bacteremia; statistical analysis of data using SPSS13.0 software. The results of blood cultures were isolated from 69 strains of Candida, the Candida albicans accounted for 50.7% of 35 strains, 12 strains of Candida glabrada accounted for 17.4%. 6 strains of Candida parapsilosis accounted for 8.7%, Candida tropicalis 5 strains accounted for 7.2%, by single factor analysis and multivariate logistic regression analysis showed that septic shock (OR= 8.601,95%CI 1.756~42.120 (OR=32.393,95%CI), bacteremia associated with bacterial 3.686~284.657), hypoproteinemia (OR=25.496,95%CI 2.876~226.067) is an independent risk factor for death. Conclusion ICU Candida bacteremia with Candida albicans, high fatality rate, should take corresponding treatment measures according to the risk factors.
