【摘要】：目的探討"三明治"式支氣管動(dòng)脈栓塞術(shù)(BAE)治療咯血的臨床療效。方法收集2010年3月—2012年3月進(jìn)行BAE治療的30例不同程度的咯血患者,術(shù)后對所有病例進(jìn)行為期1~3年的追蹤隨訪。用SPSS17.0統(tǒng)計(jì)軟件對咯血復(fù)發(fā)率、遠(yuǎn)期控制率進(jìn)行估算,并分析BAE治療方法、咳血復(fù)發(fā)原因及治療措施等。結(jié)果 "三明治"式BAE后止血的即刻有效率100%(30/30),有效率30.0%(9/30),治愈率63.3%(19/30),復(fù)發(fā)率為6.7%(2/30),對咯血的1年控制率和估計(jì)2年控制率均為76%和68%,本組病例中2例需要再次進(jìn)行支氣管動(dòng)脈栓塞治療,造影顯示1例為漏栓責(zé)任血管(時(shí)間距上次手術(shù)時(shí)間為8 d),另1例為原左側(cè)已栓塞支氣管動(dòng)脈再次開放,血管增粗、扭曲(距上次栓塞時(shí)間為7個(gè)月),2次栓塞后病變血管均未再顯影,患者停止咯血且未復(fù)發(fā)。結(jié)論 "三明治"式支氣管動(dòng)脈栓塞術(shù)治療大咯血安全、有效,咯血復(fù)發(fā)的主要原因?yàn)槁┧皞?cè)支循環(huán)的形成。
[Abstract]:Objective to investigate the clinical effect of sandwich bronchial artery embolism (BAE) in the treatment of hemoptysis. Methods from March 2010 to March 2012, 30 patients with hemoptysis treated with BAE were followed up for 1 to 3 years. The recurrence rate and long-term control rate of hemoptysis were estimated by SPSS17.0 statistical software, and the treatment methods of BAE, the causes of hemoptysis recurrence and the treatment measures were analyzed.
