【摘要】：目的研究生物膜表達水平與銅綠假單胞菌引起的呼吸機相關(guān)性肺炎患者預(yù)后的關(guān)系以及相關(guān)危險因素分析,從而指導(dǎo)臨床改善患者預(yù)后。方法收集2013年1月-2014年12月醫(yī)院銅綠假單胞菌致呼吸機相關(guān)性肺炎患者34例,采用結(jié)晶紫法測定細菌生物膜的表達;收集患者臨床資料,包括生命體征、基礎(chǔ)疾病、實驗室檢查、通氣參數(shù)、治療方案及預(yù)后。結(jié)果 34例患者中,生物膜高表達8例,低表達26例,兩組之間死亡率差異無統(tǒng)計學(xué)意義;死亡11例;統(tǒng)計分析結(jié)果顯示白細胞數(shù)、休克、意識狀態(tài)、TPN膳食、預(yù)防性抗菌藥物治療在呼吸機相關(guān)性肺炎預(yù)后上差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論生物膜表達水平與銅綠假單胞菌呼吸機相關(guān)性肺炎預(yù)后無顯著相關(guān)性。白細胞數(shù)、休克、昏迷、TPN膳食、預(yù)防性抗菌藥物治療是呼吸機相關(guān)性肺炎患者的死亡相關(guān)危險因素。
[Abstract]:Objective to analyze the relationship between the expression level of membrane and the prognosis of ventilator-associated pneumonia patients caused by Pseudomonas aeruginosa and the analysis of risk factors, so as to guide the clinical improvement of the prognosis of patients with ventilator-associated pneumonia. Methods 34 patients with ventilator-associated pneumonia caused by Pseudomonas aeruginosa were collected from January 2013 to December 2014. The expression of bacterial biofilm was measured by crystal violet method. Collect clinical data of patients, including vital signs, basic diseases, laboratory tests, ventilation parameters, treatment regimen and prognosis. Results there were 8 cases with high expression of biofilm and 26 cases with low expression of biomembrane in 34 patients. There was no significant difference in mortality between the two groups. Statistical analysis showed that there were significant differences in the prognosis of ventilator-associated pneumonia in white blood cell count, shock, state of consciousness, TPN diet and prophylactic antibiotic therapy (P0.05). Conclusion there is no significant correlation between the expression of biofilm and the prognosis of ventilator-associated pneumonia of Pseudomonas aeruginosa. Leukocyte count, shock, coma, TPN diet and prophylactic antimicrobial therapy were risk factors for death in patients with ventilator-associated pneumonia.
