【摘要】：目的探索呼吸機(jī)相關(guān)性肺炎(VAP)非抗菌藥物綜合干預(yù)策略在臨床應(yīng)用的效果。方法選擇2014年1月-2015年12月入住重癥監(jiān)護(hù)病房氣管插管有創(chuàng)呼吸機(jī)輔助通氣的患者開展統(tǒng)計(jì);其中將2014年1月-12月患者按呼吸機(jī)的護(hù)理常規(guī)進(jìn)行護(hù)理作為對(duì)照組,共3 858人;將2015年1月-12月的患者,共4 080人,實(shí)施依從性監(jiān)測(cè)作為實(shí)驗(yàn)組;通過(guò)開展呼吸機(jī)相關(guān)性肺炎預(yù)防與控制措施的依從性監(jiān)測(cè)(洗手、口腔衛(wèi)生、半臥位等依從性),同時(shí)監(jiān)測(cè)呼吸機(jī)相關(guān)性肺炎的發(fā)生率。結(jié)果用氯已定每天三次口腔護(hù)理完成率達(dá)100%、半臥位依從性達(dá)98.82%、洗手的依從性達(dá)到85%以上;2014年收住患者中呼吸機(jī)相關(guān)性肺炎發(fā)生率25.7‰,2015年收治患者中呼吸機(jī)相關(guān)性肺炎的發(fā)生率13.97‰。結(jié)論呼吸機(jī)相關(guān)性肺炎非抗菌藥物綜合干預(yù)策略可提高洗手、口腔衛(wèi)生、半臥位等依從性,從而顯著降低呼吸機(jī)相關(guān)肺炎的發(fā)生率。
[Abstract]:Objective to explore the clinical application of (VAP) non-antibacterial intervention strategy in ventilator-associated pneumonia. Methods from January 2014 to December 2015, the patients who were admitted to intensive care unit (ICU) with invasive ventilator-assisted ventilation were selected for statistics. Among them, 3,858 patients were treated as control group according to the nursing routine of ventilator from January to December 2014, and 4,080 patients from January to December 2015 were used as experimental group. Compliance monitoring of ventilator-associated pneumonia prevention and control measures (hand washing, oral hygiene, semi-supine position, etc.) was carried out, and the incidence of ventilator-associated pneumonia was monitored at the same time. Results the completion rate of oral care three times a day with chlorhexidine was 100%, the compliance of semi-lying position was 98.82%, and the compliance of hand washing was more than 85%.
