下腔靜脈濾器置入預防肺栓塞的臨床分析

【摘要】:目的: 探討下腔靜脈濾器置入術在預防肺栓塞中的應用價值 方法: 回顧性分析2012年1月至2013年1月收治的380例下肢深靜脈血栓形成(Deep vein thrombosis,DVT)患者的臨床資料,在告知肺栓塞(Pulmonaryembolism,PE)相關風險后尊重其意愿選擇放置或者不放置下腔靜脈濾器(Inferior Vena Cava Filter,IVCF)。選擇放置IVCF為治療組(I組)共179例,未放置IVCF患者為對照組(II組)共201例,所有兩組患者出院后通過定期門診復查進行隨訪,隨訪3-6個月。通過復查明確有無肺栓塞發(fā)生及下肢DVT復發(fā),結合病案資料和隨訪結果比較各組PE發(fā)生率及下肢DVT復發(fā)率。 結果: I組中失回訪1例,II組中失回訪4例;II組中因惡性腫瘤死亡2例,不納入統(tǒng)計。 (1)PE發(fā)生率:I組中PE例數(shù)為2,,死亡0例,PE發(fā)生率為1.12%; II組中PE發(fā)生例數(shù)為14例,死亡1例,PE發(fā)生率為7.18%。I組PE發(fā)生率低于II組,兩者比較具有統(tǒng)計學意義(P 0.05)。 (2)DVT復發(fā)率:I組中DVT復發(fā)例數(shù)為13,發(fā)生率為6.67%;II組中DVT復發(fā)例數(shù)為5,復發(fā)率為2.56%。I組DVT復發(fā)率高于于II組,二者比較具有統(tǒng)計學差異(P<0.05)。 結論: 下腔靜脈濾器置入可降低下肢深靜脈血栓致肺栓塞的發(fā)生率,但會增加下肢DVT復發(fā)率。
[Abstract]:Objective: to evaluate the value of inferior vena cava filter (IVC) in the prevention of pulmonary embolism. Methods: from January 2012 to January 2013, 380cases of deep venous thrombosis of lower extremities (Deep vein thrombosis,) were retrospectively analyzed. The clinical data of patients with DVT respected their desire to place or not place the inferior vena cava filter (Inferior Vena Cava Filter,IVCF) after informing them of the risk associated with pulmonary embolism (Pulmonaryembolism,PE). A total of 179 patients were treated with IVCF (group I) and 201 patients without IVCF (II group). All the patients in the two groups were followed up by regular out-patient follow-up for 3 months and 6 months after discharge. The incidence of pulmonary embolism and the recurrence of DVT in lower extremities were determined by reexamination. The incidence of PE and the recurrence rate of DVT in lower extremities were compared with the clinical data and follow-up results. Results: 1 case in group I, 4 cases in group II and 2 cases died of malignant tumor in group II were not included in the statistics. (1) the incidence of PE: in group I, the number of PE cases was 2, the death rate was 0, and the incidence of PE was 1.12%. There were 14 cases of PE and 1 case of death in II group. The incidence of PE in group I (7.18%) was lower than that in group II (P 0.05). (2) the recurrence rate of DVT: in group I, the number of cases of DVT recurrence was 13, the incidence rate was 6.67%; The recurrence rate of DVT in II group was 5, and the recurrence rate of DVT in 2.56% I group was higher than that in II group. There was significant difference between the two groups (P < 0.05). Conclusion: the placement of inferior vena cava filter can reduce the incidence of pulmonary embolism caused by deep venous thrombosis in lower extremities, but it can increase the recurrence rate of DVT in lower extremities.
