重癥監(jiān)護(hù)病房鮑氏不動桿菌致呼吸機(jī)相關(guān)性肺炎的危險因素分析及干預(yù)對策

【摘要】:目的多元回歸分析重癥監(jiān)護(hù)病房(ICU)鮑氏不動桿菌(ABA)致呼吸機(jī)相關(guān)性肺炎的危險因素,制定相關(guān)干預(yù)對策。方法選擇2013年3月-2016年3月醫(yī)院ICU治療ABA致呼吸機(jī)相關(guān)性肺炎患者126例進(jìn)行研究,統(tǒng)計感染ABA的藥敏性,并分析ICU ABA致呼吸機(jī)相關(guān)性肺炎發(fā)生的危險因素,進(jìn)行統(tǒng)計分析。結(jié)果ABA對氨芐西林、慶大霉素的耐藥率較高達(dá)90.00%以上,對亞胺培南和美羅培南最為敏感,耐藥率均20.00%,其中85株ABA顯示對≥3種抗菌藥物耐藥為泛耐藥;單因素分析顯示,ICU泛耐藥ABA呼吸機(jī)相關(guān)性肺炎的發(fā)生與住院時間、機(jī)械通氣方式、使用糖皮質(zhì)激素、留置胃管、使用碳青霉烯類抗菌藥物及使用≥3種抗菌藥物呈明顯相關(guān)性(χ2=20.292、10.414、7.524、7.810、9.760和11.635,P0.05);logistic回歸模型分析顯示,機(jī)械通氣方式、留置胃管、使用≥3種抗菌藥物是ICU泛耐藥ABA呼吸機(jī)相關(guān)性肺炎發(fā)生的獨(dú)立危險因素(OR=6.527、6.337、5.467,P0.05)。結(jié)論 ICU呼吸機(jī)相關(guān)性肺炎ABA泛耐藥現(xiàn)象嚴(yán)重,機(jī)械通氣方式、留置胃管、使用≥3種抗菌藥物是其發(fā)生的獨(dú)立危險因素,醫(yī)護(hù)人員應(yīng)避免ABA感染和耐藥的進(jìn)一步傳播。

Methods 126 patients with ventilator-associated pneumonia caused by ABA were treated with ICU from March 2013 to March 2016.
