肺動脈栓塞的危險(xiǎn)分級與多項(xiàng)指標(biāo)的臨床相關(guān)性應(yīng)用

【摘要】:目的評估血漿D二聚體(DD)等血凝標(biāo)志物、心肌損傷標(biāo)志物、血?dú)夥治龅仍诜蝿用}栓塞(PE)危險(xiǎn)分層診斷及預(yù)后的臨床價(jià)值。方法 201例PE患者其中高危組52例,中危組88例,低危組61例,治療組67例,正常對照組38例。分析多項(xiàng)指標(biāo)的表達(dá)水平。結(jié)果 PE者高危組、中危組和低危組的DD、纖維蛋白原(Fib)、N末端腦鈉肽(NT-pro BNP)水平、凝血酶原時(shí)間(PT)、氧分壓(PO_2)與治療組和或正常對照組比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05),而除低危組外,高危組與中危組的肌鈣蛋白I(cTnⅠ)、肌酸激酶同工酶(CK-MB)、國際標(biāo)準(zhǔn)化比值(INR)、凝血酶原活動度(PTA)和呼吸指數(shù)(RI)比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05),二氧化碳分壓(PCO_2)僅中危組差異具有統(tǒng)計(jì)學(xué)意義(P0.05),但肌酸激酶(CK)和血氧飽和度(SpO_2)差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論聯(lián)合多項(xiàng)指標(biāo)不僅可作為肺動脈栓塞的危險(xiǎn)分層鑒別診斷指標(biāo),其濃度的變化有助于判斷預(yù)后及指導(dǎo)治療。
[Abstract]:Objective to evaluate the clinical value of plasma D-dimer (DD), myocardial injury marker and blood gas analysis in the risk stratification diagnosis and prognosis of pulmonary embolism (PE). Methods there were 52 cases in high risk group, 88 cases in middle risk group, 61 cases in low risk group, 67 cases in treatment group and 38 cases in normal control group. The expression level of multiple indexes was analyzed.
