【摘要】：目的： 研究慢性阻塞性肺疾病(COPD)患者夜間睡眠時(shí)低氧血癥發(fā)生的可預(yù)測(cè)因素,以利早期診斷,早期治療,延緩并發(fā)癥的出現(xiàn)。 方法： 對(duì)安徽醫(yī)科大學(xué)第三附屬醫(yī)院呼吸內(nèi)科2010年6月至2011年12月確診的45例COPD患者(排除睡眠呼吸暫停低通氣綜合征)的白天肺功能、動(dòng)脈血?dú)饧耙归g血氧水平與正常對(duì)照組(30名)進(jìn)行比較和分析。 結(jié)果： COPD患者夜間平均血氧飽和度(MSaO2)、夜間最低血氧飽和度(LSaO2)，，較正常人均有明顯下降(P0.05)。從血氧分布情況來(lái)看,COPD患者睡眠時(shí)動(dòng)脈血氧飽和度(Sa02)90%的時(shí)間占睡眠時(shí)間百分比(T90)亦顯著高于正常人(P0.01)。相關(guān)分析表明,日間Sa02、一秒鐘用力呼氣容積(FEV1)絕對(duì)值與夜間MSaO2相關(guān)(r值依次0.704,0.435,P0.01)。建立了夜間血氧水平的預(yù)測(cè)公式：MSaO2(%)=0.536×日間Sa02(%)+2.35×FEV1絕對(duì)值(L)+38.805。日間SaO2、FEV1絕對(duì)值與夜間睡眠時(shí)血氧飽和度分布有關(guān)。 結(jié)論： COPD患者睡眠時(shí)血氧飽和度較正常人出現(xiàn)明顯下降,此時(shí)的血氧水平和分布可能通過(guò)日間SaO2、FEV1絕對(duì)值進(jìn)行預(yù)測(cè)。
[Abstract]:Objective: to study the predictive factors of hypoxemia during night sleep in patients with chronic obstructive pulmonary disease (COPD) in order to facilitate early diagnosis and treatment and delay the occurrence of complications. Methods: the daytime lung function of 45 patients with COPD (excluding sleep apnea hypopnea syndrome) diagnosed in Department of Respiratory Medicine, third affiliated Hospital of Anhui Medical University from June 2010 to December 2011 was studied. Arterial blood gas and nocturnal blood oxygen levels were compared and analyzed with 30 normal controls. Results: the nocturnal mean oxygen saturation (MSaO2) and nocturnal minimum oxygen saturation (LSaO2) in COPD patients were significantly lower than those in normal controls (P0.05). According to the distribution of blood oxygen, the percentage of arterial oxygen saturation (Sa02) to sleep time (T90) in patients with COPD was significantly higher than that in normal subjects (P0.01). Correlation analysis showed that the absolute value of forced expiratory volume (FEV1) in daytime Sa02, was correlated with MSaO2 at night (r = 0.704 鹵0.435, P0.01). The prediction formula of nocturnal blood oxygen level was established: MSaO2 (%) = 0.536 脳 day Sa02 (%) 2.35 脳 FEV1 absolute value (L) 38.805. The absolute value of daytime SaO2,FEV1 is related to the distribution of oxygen saturation during nighttime sleep. Conclusion: the level and distribution of blood oxygen in patients with COPD may be predicted by the absolute value of daytime SaO2,FEV1.
