脈沖振蕩技術(shù)用于矽肺患者肺功能評(píng)價(jià)的臨床應(yīng)用價(jià)值

【摘要】:目的探討脈沖振蕩技術(shù)(IOS)用于矽肺患者肺功能評(píng)價(jià)的臨床應(yīng)用價(jià)值。方法對(duì)63例矽肺患者和20例健康者依次進(jìn)行IOS及PFT檢測(cè),分析兩種肺功能各項(xiàng)指標(biāo)之間的差異、相關(guān)性。結(jié)果 (1)隨著矽肺期別上升,FVC、FEV1、FEV1/FVC呈下降趨勢(shì),叁期矽肺較其他各組均存在顯著差異(P0.05)。(2)矽肺組與健康對(duì)照組比較,IOS參數(shù)Z5、R5-R20、X5、Fres、AX明顯升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);壹期、貳期矽肺組與健康對(duì)照組比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);叁期矽肺與其他各組比較均存在顯著差異(P0.05)。(3)按通氣功能阻塞程度將矽肺組分為無阻塞和輕度、中度、重度阻塞4組,與對(duì)照組比較發(fā)現(xiàn),隨阻塞程度上升,各IOS參數(shù)結(jié)果亦呈上升趨勢(shì),且各參數(shù)組間比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。(4)除R20外,其他各IOS參數(shù)均與FEV1、FEV1/FVC顯著相關(guān)(P0.01),其中X5為正相關(guān),其他為負(fù)相關(guān)。比較相關(guān)性大小,FEV1/FVC與IOS相關(guān)性更高。結(jié)論 (1)IOS參數(shù)如Z5、R5-R20、X5、Fres、AX對(duì)發(fā)現(xiàn)早期(壹期)矽肺肺功能異常有一定臨床價(jià)值;(2)IOS和肺通氣功能具有良好的相關(guān)性,其敏感性似優(yōu)于PFT,可作為矽肺患者肺功能評(píng)價(jià)的補(bǔ)充。
[Abstract]:Objective to evaluate the clinical value of pulsed oscillatory technique (IOS) in the evaluation of pulmonary function in silicosis patients. Methods IOS and PFT were detected in 63 silicosis patients and 20 healthy controls, and the difference and correlation between the two indexes were analyzed. Results (1) with the increase of silicosis stage, FEV1 / FVC decreased, and there was significant difference between the three stages of silicosis compared with other groups (P0.05). (2). The iOS parameters of silicosis group and healthy control group were significantly higher than those of healthy control group (P0.05), the difference was statistically significant (P0.05). There were significant differences between the two groups (P0.05), and the third stage silicosis was significantly different from the other groups (P0.05). (3) according to the degree of ventilation function obstruction, the silicosis group was divided into 4 groups: non-obstructive group and mild, moderate and severe obstructive group. Compared with the control group, with the increase of the degree of obstruction, the results of each IOS parameter also showed an upward trend, and the differences between the two groups were statistically significant (P0.05). (4), except R20, all other IOS parameters were significantly correlated with FEV1,FEV1/FVC (P0.01), in which X5 was positively correlated. Others were negatively correlated. The correlation between FEV1 / FVC and IOS was higher. Conclusion (1) IOS parameters such as Z _ 5N R _ 5-R _ (20) X _ (5) FresAX have a certain clinical value in detecting early (stage one) silicosis dysfunction, (2) IOS has a good correlation with pulmonary ventilation function, and its sensitivity seems to be better than that of PFT, as a supplement to pulmonary function evaluation in silicosis patients.
