【摘要】：目的評(píng)價(jià)結(jié)核感染T細(xì)胞斑點(diǎn)試驗(yàn)(T-SPOT.TB)診斷艾滋病合并肺結(jié)核的臨床應(yīng)用價(jià)值,以利于疾病診斷與鑒別診斷。方法選取45例合并肺結(jié)核的艾滋病患者(研究組)及30例未感染肺結(jié)核的艾滋病患者(對(duì)照組)行T-SPOT.TB檢測(cè),并同時(shí)行結(jié)核菌素純蛋白衍生物(PPD)試驗(yàn)。結(jié)果研究組T-SPOT.TB陽(yáng)性率為80.0%,顯著高于對(duì)照組13.3%(χ2=32.14,P0.01),PPD試驗(yàn)在兩組間的陽(yáng)性率分別為13.3%和6.7%;研究組CD4+T細(xì)胞計(jì)數(shù)≥200/μl及200/μl的患者中T-SPOT.TB檢測(cè)陽(yáng)性率分別為86.7%和76.7%,差異無統(tǒng)計(jì)學(xué)意義;T-SPOT.TB檢測(cè)艾滋病合并肺結(jié)核的敏感性為80.0%、特異性為76.7%,陽(yáng)性似然比6.0,陰性似然比0.1;在PPD試驗(yàn)中分別為13.3%、93.3%、2.0、0.9。結(jié)論 T-SPOT.TB對(duì)艾滋病合并肺結(jié)核具有較高的診斷效能。
[Abstract]:Objective to evaluate the clinical value of T cell spot test (T-SPOT.TB) in the diagnosis of AIDS complicated with pulmonary tuberculosis. Methods T-SPOT.TB was detected in 45 AIDS patients with pulmonary tuberculosis (study group) and 30 AIDS patients without pulmonary tuberculosis (control group), and the PPD test was performed at the same time. Results the positive rate of T-SPOT.TB in the study group was 80.0, which was significantly higher than that in the control group (13.3%) (蠂 ~ (2) 2) (32.14 / P0.01).
