【摘要】：目的探討血清降鈣素原(PCT)監(jiān)測(cè)對(duì)接受門診治療的老年呼吸機(jī)相關(guān)性肺炎(VAP)患者使用的抗生素指導(dǎo)意義。方法選擇接受門診治療的老年VAP患者59例,根據(jù)隨機(jī)對(duì)照原則分為PCT組29例,常規(guī)治療組30例。入組患者均監(jiān)測(cè)血清PCT水平,在進(jìn)行常規(guī)治療的同時(shí),常規(guī)治療組根據(jù)我國(guó)VAP治療指南選擇抗生素治療方案;PCT組在血清PCT≥0.25μg/L時(shí)使用抗生素治療,PCT0.25μg/L時(shí)不予抗生素治療,復(fù)診時(shí)亦按照此標(biāo)準(zhǔn)決定是否繼續(xù)使用抗生素。觀察上述兩組的相關(guān)臨床指標(biāo)及抗生素使用率。結(jié)果 PCT組治療后血清PCT水平低于治療前。PCT監(jiān)測(cè)組的抗生素使用率低于常規(guī)治療組,兩組患者的預(yù)后無明顯差異(P0.05)。結(jié)論門診接受治療的VAP患者可以依據(jù)PCT水平指導(dǎo)抗生素應(yīng)用,能夠減少抗生素使用率,縮短抗生素療程,降低抗生素費(fèi)用。
[Abstract]:Objective to investigate the significance of serum procalcitonin (PCT) monitoring in the use of antibiotics in elderly patients with ventilator-associated pneumonia (VAP). Methods 59 elderly VAP patients received outpatient treatment were randomly divided into PCT group (29 cases) and routine treatment group (30 cases). Serum PCT levels were monitored in all patients. While routine therapy was carried out, the routine treatment group selected antibiotics according to our country's VAP treatment guidelines.
